Scalable Stewardship of an LLM-Assisted Clinical Benchmark with Physician Oversight

Reference labels for machine-learning benchmarks are increasingly synthesized with LLM assistance, but their reliability remains underexamined. We audit MedCalc-Bench, a clinical benchmark for medical score computation whose labels were partly derived with LLM assistance, and develop a scalable physician-in-the-loop stewardship pipeline to reassess them. At least 27% of test labels are likely erroneous or incomputable. On a 50-instance subset validated by physicians, our recomputed labels agree with physician ground truth 74% of the time (95% CI, 60-84%) versus 20% for the originals (95% CI, 11-33%). Using original labels to evaluate frontier LLMs underestimates accuracy by 16-23 percentage points. In a controlled reinforcement-learning experiment, a model trained on recomputed labels outperforms one trained on originals by 13.5 percentage points (95% CI, 10.6-16.6%) on physician-labeled instances, and this advantage extends to related medical tasks. LLM-assisted benchmarks can propagate systematic errors into both evaluation and post-training unless actively stewarded.

VeriFact: Verifying Facts in LLM-Generated Clinical Text with Electronic Health Records

Methods to ensure factual accuracy of text generated by large language models (LLM) in clinical medicine are lacking. VeriFact is an artificial intelligence system that combines retrieval-augmented generation and LLM-as-a-Judge to verify whether LLM-generated text is factually supported by a patient's medical history based on their electronic health record (EHR). To evaluate this system, we introduce VeriFact-BHC, a new dataset that decomposes Brief Hospital Course narratives from discharge summaries into a set of simple statements with clinician annotations for whether each statement is supported by the patient's EHR clinical notes. Whereas highest agreement between clinicians was 88.5%, VeriFact achieves up to 92.7% agreement when compared to a denoised and adjudicated average human clinican ground truth, suggesting that VeriFact exceeds the average clinician's ability to fact-check text against a patient's medical record. VeriFact may accelerate the development of LLM-based EHR applications by removing current evaluation bottlenecks.

Synthetic Patients: Simulating Difficult Conversations with Multimodal Generative AI for Medical Education

Problem: Effective patient-centered communication is a core competency for physicians. However, both seasoned providers and medical trainees report decreased confidence in leading conversations on sensitive topics such as goals of care or end-of-life discussions. The significant administrative burden and the resources required to provide dedicated training in leading difficult conversations has been a long-standing problem in medical education. Approach: In this work, we present a novel educational tool designed to facilitate interactive, real-time simulations of difficult conversations in a video-based format through the use of multimodal generative artificial intelligence (AI). Leveraging recent advances in language modeling, computer vision, and generative audio, this tool creates realistic, interactive scenarios with avatars, or "synthetic patients." These synthetic patients interact with users throughout various stages of medical care using a custom-built video chat application, offering learners the chance to practice conversations with patients from diverse belief systems, personalities, and ethnic backgrounds. Outcomes: While the development of this platform demanded substantial upfront investment in labor, it offers a highly-realistic simulation experience with minimal financial investment. For medical trainees, this educational tool can be implemented within programs to simulate patient-provider conversations and can be incorporated into existing palliative care curriculum to provide a scalable, high-fidelity simulation environment for mastering difficult conversations. Next Steps: Future developments will explore enhancing the authenticity of these encounters by working with patients to incorporate their histories and personalities, as well as employing the use of AI-generated evaluations to offer immediate, constructive feedback to learners post-simulation.
