EyeWorld: A Generative World Model of Ocular State and Dynamics

Ophthalmic decision-making depends on subtle lesion-scale cues interpreted across multimodal imaging and over time, yet most medical foundation models remain static and degrade under modality and acquisition shifts. Here we introduce EyeWorld, a generative world model that conceptualizes the eye as a partially observed dynamical system grounded in clinical imaging. EyeWorld learns an observation-stable latent ocular state shared across modalities, unifying fine-grained parsing, structure-preserving cross-modality translation and quality-robust enhancement within a single framework. Longitudinal supervision further enables time-conditioned state transitions, supporting forecasting of clinically meaningful progression while preserving stable anatomy. By moving from static representation learning to explicit dynamical modeling, EyeWorld provides a unified approach to robust multimodal interpretation and prognosis-oriented simulation in medicine.

Empowering Locally Deployable Medical Agent via State Enhanced Logical Skills for FHIR-based Clinical Tasks

While Large Language Models demonstrate immense potential as proactive Medical Agents, their real-world deployment is severely bottlenecked by data scarcity under privacy constraints. To overcome this, we propose State-Enhanced Logical-Skill Memory (SELSM), a training-free framework that distills simulated clinical trajectories into entity-agnostic operational rules within an abstract skill space. During inference, a Query-Anchored Two-Stage Retrieval mechanism dynamically fetches these entity-agnostic logical priors to guide the agent's step-by-step reasoning, effectively resolving the state polysemy problem. Evaluated on MedAgentBench -- the only authoritative high-fidelity virtual EHR sandbox benchmarked with real clinical data -- SELSM substantially elevates the zero-shot capabilities of locally deployable foundation models (30B--32B parameters). Notably, on the Qwen3-30B-A3B backbone, our framework completely eliminates task chain breakdowns to achieve a 100\% completion rate, boosting the overall success rate by an absolute 22.67\% and significantly outperforming existing memory-augmented baselines. This study demonstrates that equipping models with a dynamically updatable, state-enhanced cognitive scaffold is a privacy-preserving and computationally efficient pathway for local adaptation of AI agents to clinical information systems. While currently validated on FHIR-based EHR interactions as an initial step, the entity-agnostic design of SELSM provides a principled foundation toward broader clinical deployment.

EyeAgent: An Agentic AI System for Multimodal Clinical Decision Support in Ophthalmology

Artificial intelligence has shown promise in medical imaging, yet most existing systems lack flexibility, interpretability, and adaptability - challenges especially pronounced in ophthalmology, where diverse imaging modalities are essential. We present EyeAgent, the first agentic AI framework for comprehensive and interpretable clinical decision support in ophthalmology. Using a large language model (DeepSeek-V3) as its central reasoning engine, EyeAgent interprets user queries and dynamically orchestrates 53 validated ophthalmic tools across 23 imaging modalities for diverse tasks including classification, segmentation, detection, image/report generation, and quantitative analysis. Stepwise ablation analysis demonstrated a progressive improvement in diagnostic accuracy, rising from a baseline of 69.71% (using only 5 general tools) to 80.79% when the full suite of 53 specialized tools was integrated. In an expert rating study on 200 real-world clinical cases, EyeAgent achieved 93.7% tool selection accuracy and received expert ratings of more than 88% across accuracy, completeness, safety, reasoning, and interpretability. In human-AI collaboration, EyeAgent matched or exceeded the performance of senior ophthalmologists and, when used as an assistant, improved overall diagnostic accuracy by 18.51% and report quality scores by 19%, with the greatest benefit observed among junior ophthalmologists. These findings establish EyeAgent as a scalable and trustworthy AI framework for ophthalmology and provide a blueprint for modular, multimodal, and clinically aligned next-generation AI systems.

Benchmarking Large Multimodal Models for Ophthalmic Visual Question Answering with OphthalWeChat

Purpose: To develop a bilingual multimodal visual question answering (VQA) benchmark for evaluating VLMs in ophthalmology. Methods: Ophthalmic image posts and associated captions published between January 1, 2016, and December 31, 2024, were collected from WeChat Official Accounts. Based on these captions, bilingual question-answer (QA) pairs in Chinese and English were generated using GPT-4o-mini. QA pairs were categorized into six subsets by question type and language: binary (Binary_CN, Binary_EN), single-choice (Single-choice_CN, Single-choice_EN), and open-ended (Open-ended_CN, Open-ended_EN). The benchmark was used to evaluate the performance of three VLMs: GPT-4o, Gemini 2.0 Flash, and Qwen2.5-VL-72B-Instruct. Results: The final OphthalWeChat dataset included 3,469 images and 30,120 QA pairs across 9 ophthalmic subspecialties, 548 conditions, 29 imaging modalities, and 68 modality combinations. Gemini 2.0 Flash achieved the highest overall accuracy (0.548), outperforming GPT-4o (0.522, P < 0.001) and Qwen2.5-VL-72B-Instruct (0.514, P < 0.001). It also led in both Chinese (0.546) and English subsets (0.550). Subset-specific performance showed Gemini 2.0 Flash excelled in Binary_CN (0.687), Single-choice_CN (0.666), and Single-choice_EN (0.646), while GPT-4o ranked highest in Binary_EN (0.717), Open-ended_CN (BLEU-1: 0.301; BERTScore: 0.382), and Open-ended_EN (BLEU-1: 0.183; BERTScore: 0.240). Conclusions: This study presents the first bilingual VQA benchmark for ophthalmology, distinguished by its real-world context and inclusion of multiple examinations per patient. The dataset reflects authentic clinical decision-making scenarios and enables quantitative evaluation of VLMs, supporting the development of accurate, specialized, and trustworthy AI systems for eye care.

Towards Harnessing the Collaborative Power of Large and Small Models for Domain Tasks

Large language models (LMs) offer broad generalization capabilities but require vast amounts of data and computational resources for domain-specific tasks; small models (SMs), in contrast, are more efficient and tailored to specific domains yet lack general-purpose coverage. Taking a collaborative approach, where large and small models work synergistically, can accelerate the adaptation of LLMs to private domains and unlock new potential in AI. This survey presents a comprehensive overview of recent advances and challenges in harnessing the collaborative power of large and small models for private-domain adaptation. It specifically focuses on the unique constraints of cross-boundary environments, where models belong to distinct parties, and examines the resulting tensions among data privacy, model security, integrity, and resource limitations. By analyzing the information flow between distinct model and data stakeholders, we propose a unified taxonomy that classifies research into three primary directions: downward knowledge transfer (LM to SM), upward knowledge transfer (SM to LM), and inference-time collaboration across parties. Drawing on this taxonomy, we analyze the core challenges inherent to cross-boundary information exchange, including data-privacy, model-security, and integrity threats as well as efficiency constraints, and synthesize these into a multi-objective optimization problem that governs practical deployment. Finally, we review key open challenges inherent to such hybrid approaches and outline promising directions for future research. By offering a principled, boundary-centric view of this rapidly evolving landscape, this survey aims to serve as a structured resource for researchers and practitioners advancing privacy-aware, resource-efficient AI deployment.

Doubly-Bounded Queue for Constrained Online Learning: Keeping Pace with Dynamics of Both Loss and Constraint

We consider online convex optimization with time-varying constraints and conduct performance analysis using two stringent metrics: dynamic regret with respect to the online solution benchmark, and hard constraint violation that does not allow any compensated violation over time. We propose an efficient algorithm called Constrained Online Learning with Doubly-bounded Queue (COLDQ), which introduces a novel virtual queue that is both lower and upper bounded, allowing tight control of the constraint violation without the need for the Slater condition. We prove via a new Lyapunov drift analysis that COLDQ achieves $O(T^\frac{1+V_x}{2})$ dynamic regret and $O(T^{V_g})$ hard constraint violation, where $V_x$ and $V_g$ capture the dynamics of the loss and constraint functions. For the first time, the two bounds smoothly approach to the best-known $O(T^\frac{1}{2})$ regret and $O(1)$ violation, as the dynamics of the losses and constraints diminish. For strongly convex loss functions, COLDQ matches the best-known $O(\log{T})$ static regret while maintaining the $O(T^{V_g})$ hard constraint violation. We further introduce an expert-tracking variation of COLDQ, which achieves the same performance bounds without any prior knowledge of the system dynamics. Simulation results demonstrate that COLDQ outperforms the state-of-the-art approaches.

Survey on Knowledge Distillation for Large Language Models: Methods, Evaluation, and Application

Large Language Models (LLMs) have showcased exceptional capabilities in various domains, attracting significant interest from both academia and industry. Despite their impressive performance, the substantial size and computational demands of LLMs pose considerable challenges for practical deployment, particularly in environments with limited resources. The endeavor to compress language models while maintaining their accuracy has become a focal point of research. Among the various methods, knowledge distillation has emerged as an effective technique to enhance inference speed without greatly compromising performance. This paper presents a thorough survey from three aspects: method, evaluation, and application, exploring knowledge distillation techniques tailored specifically for LLMs. Specifically, we divide the methods into white-box KD and black-box KD to better illustrate their differences. Furthermore, we also explored the evaluation tasks and distillation effects between different distillation methods, and proposed directions for future research. Through in-depth understanding of the latest advancements and practical applications, this survey provides valuable resources for researchers, paving the way for sustained progress in this field.

A Novel Optimized Asynchronous Federated Learning Framework

Federated Learning (FL) since proposed has been applied in many fields, such as credit assessment, medical, etc. Because of the difference in the network or computing resource, the clients may not update their gradients at the same time that may take a lot of time to wait or idle. That's why Asynchronous Federated Learning (AFL) method is needed. The main bottleneck in AFL is communication. How to find a balance between the model performance and the communication cost is a challenge in AFL. This paper proposed a novel AFL framework VAFL. And we verified the performance of the algorithm through sufficient experiments. The experiments show that VAFL can reduce the communication times about 51.02\% with 48.23\% average communication compression rate and allow the model to be converged faster. FedCM: A Real-time Contribution Measurement Method for Participants in Federated Learning

Federated Learning (FL) creates an ecosystem for multiple agents to collaborate on building models with data privacy consideration. The method for contribution measurement of each agent in the FL system is critical for fair credits allocation but few are proposed. In this paper, we develop a real-time contribution measurement method FedCM that is simple but powerful. The method defines the impact of each agent, comprehensively considers the current round and the previous round to obtain the contribution rate of each agent with attention aggregation. Moreover, FedCM updates contribution every round, which enable it to perform in real-time. Real-time is not considered by the existing approaches, but it is critical for FL systems to allocate computing power, communication resources, etc. Compared to the state-of-the-art method, the experimental results show that FedCM is more sensitive to data quantity and data quality under the premise of real-time. Furthermore, we developed federated learning open-source software based on FedCM. The software has been applied to identify COVID-19 based on medical images.

Experiments of Federated Learning for COVID-19 Chest X-ray Images

AI plays an important role in COVID-19 identification. Computer vision and deep learning techniques can assist in determining COVID-19 infection with Chest X-ray Images. However, for the protection and respect of the privacy of patients, the hospital's specific medical-related data did not allow leakage and sharing without permission. Collecting such training data was a major challenge. To a certain extent, this has caused a lack of sufficient data samples when performing deep learning approaches to detect COVID-19. Federated Learning is an available way to address this issue. It can effectively address the issue of data silos and get a shared model without obtaining local data. In the work, we propose the use of federated learning for COVID-19 data training and deploy experiments to verify the effectiveness. And we also compare performances of four popular models (MobileNet, ResNet18, MoblieNet, and COVID-Net) with the federated learning framework and without the framework. This work aims to inspire more researches on federated learning about COVID-19.

An Improved Method for the Fitting and Prediction of the Number of COVID-19 Confirmed Cases Based on LSTM

New coronavirus disease (COVID-19) has constituted a global pandemic and has spread to most countries and regions in the world. By understanding the development trend of a regional epidemic, the epidemic can be controlled using the development policy. The common traditional mathematical differential equations and population prediction models have limitations for time series population prediction, and even have large estimation errors. To address this issue, we propose an improved method for predicting confirmed cases based on LSTM (Long-Short Term Memory) neural network. This work compared the deviation between the experimental results of the improved LSTM prediction model and the digital prediction models (such as Logistic and Hill equations) with the real data as reference. And this work uses the goodness of fitting to evaluate the fitting effect of the improvement. Experiments show that the proposed approach has a smaller prediction deviation and a better fitting effect. Compared with the previous forecasting methods, the contributions of our proposed improvement methods are mainly in the following aspects: 1) we have fully considered the spatiotemporal characteristics of the data, rather than single standardized data; 2) the improved parameter settings and evaluation indicators are more accurate for fitting and forecasting. 3) we consider the impact of the epidemic stage and conduct reasonable data processing for different stage.

Encryption Algorithm for TCP Session Hijacking

Distributed network of the computer and the design defects of the TCP protocol are given to the network attack to be multiplicative. Based on the simple and open assumptions of the TCP protocol in academic and collaborative communication environments, the protocol lacks secure authentication. In this paper, by adding RSA-based cryptography technology, RSA-based signature technology, DH key exchange algorithm, and HAMC-SHA1 integrity verification technology to the TCP protocol, and propose a security strategy which can effectively defend against TCP session hijacking.
