SeVeR: Selective Visual Exposure and Retrieval for 3D Medical Image Question Answering

Volumetric medical VQA requires reasoning over long and redundant 3D visual token sequences, especially in multi-sequence MRI where complementary modalities provide diverse diagnostic cues but expose the decoder to many repeated anatomical regions. To investigate reasoning under multi-sequence visual redundancy, we first introduce BreMRIs-VQA, a clinically curated breast MRI benchmark with 1.19M QA pairs from 71.0K sequences and 12.9K patients, covering both free-text and multiple-choice questions. We further propose SeVeR, a selective visual exposure framework that compresses dense volumes into modality-wise prototypes and retrieves complementary multi-level evidence with change-aware gated attention during decoding, trained with a marginal-utility self-consistency objective that suppresses unhelpful retrieval. Experiments on BreMRIs-VQA and public benchmarks show that SeVeR improves both discriminative and generative performance while exposing substantially fewer visual tokens.

D3O: Dynamic Distribution Distillation for Ordinal Regression

Ordinal regression is widely used in scenarios where labels are discrete yet inherently ordered. In practice, however, ordinal labels are often obtained by discretizing underlying continuous semantics through subjective human judgment, resulting in ambiguous boundaries and annotation noise. Such uncertainty challenges existing methods that rely on fixed supervision targets, which may reinforce biased ordering under subjective annotations. To address this limitation, we propose D3O, a dynamic distribution distillation framework that replaces static supervision with training-driven evolution of ordinal label distributions via self-distillation. Specifically, we introduce a contrastive ordinal-aware label enhancement module that leverages vision-language alignment to recover refined label distributions capturing both inter-class ambiguity and instance-level uncertainty. Furthermore, we design a CDF-based cross-layer interaction distillation mechanism to propagate cumulative ordinal structure across network hierarchy, ensuring consistent ordinal geometry in intermediate representations. Extensive experiments on four general ordinal regression tasks demonstrate that our proposed D3O consistently outperforms existing approaches, particularly under severe class imbalance and noisy supervision. These results highlight the effectiveness of dynamic supervision in learning robust ordinal representations beyond fixed targets. The code will be publicly available.

Dual-level Fuzzy Learning with Patch Guidance for Image Ordinal Regression

Ordinal regression bridges regression and classification by assigning objects to ordered classes. While human experts rely on discriminative patch-level features for decisions, current approaches are limited by the availability of only image-level ordinal labels, overlooking fine-grained patch-level characteristics. In this paper, we propose a Dual-level Fuzzy Learning with Patch Guidance framework, named DFPG that learns precise feature-based grading boundaries from ambiguous ordinal labels, with patch-level supervision. Specifically, we propose patch-labeling and filtering strategies to enable the model to focus on patch-level features exclusively with only image-level ordinal labels available. We further design a dual-level fuzzy learning module, which leverages fuzzy logic to quantitatively capture and handle label ambiguity from both patch-wise and channel-wise perspectives. Extensive experiments on various image ordinal regression datasets demonstrate the superiority of our proposed method, further confirming its ability in distinguishing samples from difficult-to-classify categories.
