The Mirage of LLM Guardrails: A Case Study in AI-Assisted Medical Note Manipulation

The rapid deployment of large language models (LLMs) in healthcare settings makes the reliability of their built-in guardrails against malicious queries a question of urgent practical consequence. Yet the robustness of these mechanisms against deliberate misuse (in the healthcare context) remains poorly understood. In this paper, we investigate this question empirically, using AI-assisted medical note manipulation as a concrete case study. We make four novel contributions. First, we develop a reproducible manipulation pipeline that takes publicly available seed medical note templates and use commercial LLMs to produce customized manipulated notes by substituting patient names, provider identities, dates, and medical conditions across multiple model families, input formats, and prompt phrasings. Second, we conduct a systematic empirical evaluation of LLM guardrail robustness for medical note manipulation. Our experimental results reveal substantial weaknesses and inconsistencies in contemporary commercial LLM guardrails, including low refusal rates for several model families. Third, we utilize a combination of automated metrics and human annotation-based metrics to assess the correctness of requested manipulations. Fourth, we conduct a user-study to assess the believability of manipulated medical notes, finding that the best manipulations are visually indistinguishable from original documents to human raters. Finally, we discuss implications for responsible guardrail design in LLMs, AI safety policies, and the broader ethics of deploying LLMs in healthcare settings.

"I Upload...All Types of Different Things to Say, the World of Blindness Is More Than What They Think It Is": A Study of Blind TikTokers' Identity Work from a Flourishing Perspective

Identity work in Human-Computer Interaction (HCI) has focused on the marginalized group to explore designs to support their asset (what they have). However, little has been explored specifically on the identity work of people with disabilities, specifically, visual impairments. In this study, we interviewed 45 BlindTokers (blind users on TikTok) from various backgrounds to understand their identity work from a positive design perspective. We found that BlindTokers leverage the affordance of the platform to create positive content, share their identities, and build the community with the desire to flourish. We proposed flourishing labor to present the work conducted by BlindTokers for their community's flourishing with implications to support the flourishing labor. This work contributes to understanding blind users' experience in short video platforms and highlights that flourishing is not just an activity for any single Blind user but also a job that needs all stakeholders, including all user groups and the TikTok platform, serious and committed contribution.

A Taxonomy of Rater Disagreements: Surveying Challenges & Opportunities from the Perspective of Annotating Online Toxicity

Toxicity is an increasingly common and severe issue in online spaces. Consequently, a rich line of machine learning research over the past decade has focused on computationally detecting and mitigating online toxicity. These efforts crucially rely on human-annotated datasets that identify toxic content of various kinds in social media texts. However, such annotations historically yield low inter-rater agreement, which was often dealt with by taking the majority vote or other such approaches to arrive at a single ground truth label. Recent research has pointed out the importance of accounting for the subjective nature of this task when building and utilizing these datasets, and this has triggered work on analyzing and better understanding rater disagreements, and how they could be effectively incorporated into the machine learning developmental pipeline. While these efforts are filling an important gap, there is a lack of a broader framework about the root causes of rater disagreement, and therefore, we situate this work within that broader landscape. In this survey paper, we analyze a broad set of literature on the reasons behind rater disagreements focusing on online toxicity, and propose a detailed taxonomy for the same. Further, we summarize and discuss the potential solutions targeting each reason for disagreement. We also discuss several open issues, which could promote the future development of online toxicity research.
