AWSnet: An Auto-weighted Supervision Attention Network for Myocardial Scar and Edema Segmentation in Multi-sequence Cardiac Magnetic Resonance Images

Multi-sequence cardiac magnetic resonance (CMR) provides essential pathology information (scar and edema) to diagnose myocardial infarction. However, automatic pathology segmentation can be challenging due to the difficulty of effectively exploring the underlying information from the multi-sequence CMR data. This paper aims to tackle the scar and edema segmentation from multi-sequence CMR with a novel auto-weighted supervision framework, where the interactions among different supervised layers are explored under a task-specific objective using reinforcement learning. Furthermore, we design a coarse-to-fine framework to boost the small myocardial pathology region segmentation with shape prior knowledge. The coarse segmentation model identifies the left ventricle myocardial structure as a shape prior, while the fine segmentation model integrates a pixel-wise attention strategy with an auto-weighted supervision model to learn and extract salient pathological structures from the multi-sequence CMR data. Extensive experimental results on a publicly available dataset from Myocardial pathology segmentation combining multi-sequence CMR (MyoPS 2020) demonstrate our method can achieve promising performance compared with other state-of-the-art methods. Our method is promising in advancing the myocardial pathology assessment on multi-sequence CMR data. To motivate the community, we have made our code publicly available via https://github.com/soleilssss/AWSnet/tree/master.

Image-Guided Navigation of a Robotic Ultrasound Probe for Autonomous Spinal Sonography Using a Shadow-aware Dual-Agent Framework

Ultrasound (US) imaging is commonly used to assist in the diagnosis and interventions of spine diseases, while the standardized US acquisitions performed by manually operating the probe require substantial experience and training of sonographers. In this work, we propose a novel dual-agent framework that integrates a reinforcement learning (RL) agent and a deep learning (DL) agent to jointly determine the movement of the US probe based on the real-time US images, in order to mimic the decision-making process of an expert sonographer to achieve autonomous standard view acquisitions in spinal sonography. Moreover, inspired by the nature of US propagation and the characteristics of the spinal anatomy, we introduce a view-specific acoustic shadow reward to utilize the shadow information to implicitly guide the navigation of the probe toward different standard views of the spine. Our method is validated in both quantitative and qualitative experiments in a simulation environment built with US data acquired from 17 volunteers. The average navigation accuracy toward different standard views achieves 5.18mm/5.25deg and 12.87mm/17.49deg in the intra- and inter-subject settings, respectively. The results demonstrate that our method can effectively interpret the US images and navigate the probe to acquire multiple standard views of the spine.

Spatio-Temporal Dynamic Inference Network for Group Activity Recognition

Group activity recognition aims to understand the activity performed by a group of people. In order to solve it, modeling complex spatio-temporal interactions is the key. Previous methods are limited in reasoning on a predefined graph, which ignores the inherent person-specific interaction context. Moreover, they adopt inference schemes that are computationally expensive and easily result in the over-smoothing problem. In this paper, we manage to achieve spatio-temporal person-specific inferences by proposing Dynamic Inference Network (DIN), which composes of Dynamic Relation (DR) module and Dynamic Walk (DW) module. We firstly propose to initialize interaction fields on a primary spatio-temporal graph. Within each interaction field, we apply DR to predict the relation matrix and DW to predict the dynamic walk offsets in a joint-processing manner, thus forming a person-specific interaction graph. By updating features on the specific graph, a person can possess a global-level interaction field with a local initialization. Experiments indicate both modules' effectiveness. Moreover, DIN achieves significant improvement compared to previous state-of-the-art methods on two popular datasets under the same setting, while costing much less computation overhead of the reasoning module.

Self Context and Shape Prior for Sensorless Freehand 3D Ultrasound Reconstruction

3D ultrasound (US) is widely used for its rich diagnostic information. However, it is criticized for its limited field of view. 3D freehand US reconstruction is promising in addressing the problem by providing broad range and freeform scan. The existing deep learning based methods only focus on the basic cases of skill sequences, and the model relies on the training data heavily. The sequences in real clinical practice are a mix of diverse skills and have complex scanning paths. Besides, deep models should adapt themselves to the testing cases with prior knowledge for better robustness, rather than only fit to the training cases. In this paper, we propose a novel approach to sensorless freehand 3D US reconstruction considering the complex skill sequences. Our contribution is three-fold. First, we advance a novel online learning framework by designing a differentiable reconstruction algorithm. It realizes an end-to-end optimization from section sequences to the reconstructed volume. Second, a self-supervised learning method is developed to explore the context information that reconstructed by the testing data itself, promoting the perception of the model. Third, inspired by the effectiveness of shape prior, we also introduce adversarial training to strengthen the learning of anatomical shape prior in the reconstructed volume. By mining the context and structural cues of the testing data, our online learning methods can drive the model to handle complex skill sequences. Experimental results on developmental dysplasia of the hip US and fetal US datasets show that, our proposed method can outperform the start-of-the-art methods regarding the shift errors and path similarities.

Flip Learning: Erase to Segment

Nodule segmentation from breast ultrasound images is challenging yet essential for the diagnosis. Weakly-supervised segmentation (WSS) can help reduce time-consuming and cumbersome manual annotation. Unlike existing weakly-supervised approaches, in this study, we propose a novel and general WSS framework called Flip Learning, which only needs the box annotation. Specifically, the target in the label box will be erased gradually to flip the classification tag, and the erased region will be considered as the segmentation result finally. Our contribution is three-fold. First, our proposed approach erases on superpixel level using a Multi-agent Reinforcement Learning framework to exploit the prior boundary knowledge and accelerate the learning process. Second, we design two rewards: classification score and intensity distribution reward, to avoid under- and over-segmentation, respectively. Third, we adopt a coarse-to-fine learning strategy to reduce the residual errors and improve the segmentation performance. Extensively validated on a large dataset, our proposed approach achieves competitive performance and shows great potential to narrow the gap between fully-supervised and weakly-supervised learning.

Style Curriculum Learning for Robust Medical Image Segmentation

The performance of deep segmentation models often degrades due to distribution shifts in image intensities between the training and test data sets. This is particularly pronounced in multi-centre studies involving data acquired using multi-vendor scanners, with variations in acquisition protocols. It is challenging to address this degradation because the shift is often not known \textit{a priori} and hence difficult to model. We propose a novel framework to ensure robust segmentation in the presence of such distribution shifts. Our contribution is three-fold. First, inspired by the spirit of curriculum learning, we design a novel style curriculum to train the segmentation models using an easy-to-hard mode. A style transfer model with style fusion is employed to generate the curriculum samples. Gradually focusing on complex and adversarial style samples can significantly boost the robustness of the models. Second, instead of subjectively defining the curriculum complexity, we adopt an automated gradient manipulation method to control the hard and adversarial sample generation process. Third, we propose the Local Gradient Sign strategy to aggregate the gradient locally and stabilise training during gradient manipulation. The proposed framework can generalise to unknown distribution without using any target data. Extensive experiments on the public M\&Ms Challenge dataset demonstrate that our proposed framework can generalise deep models well to unknown distributions and achieve significant improvements in segmentation accuracy.

Bootstrap Representation Learning for Segmentation on Medical Volumes and Sequences

In this work, we propose a novel straightforward method for medical volume and sequence segmentation with limited annotations. To avert laborious annotating, the recent success of self-supervised learning(SSL) motivates the pre-training on unlabeled data. Despite its success, it is still challenging to adapt typical SSL methods to volume/sequence segmentation, due to their lack of mining on local semantic discrimination and rare exploitation on volume and sequence structures. Based on the continuity between slices/frames and the common spatial layout of organs across volumes/sequences, we introduced a novel bootstrap self-supervised representation learning method by leveraging the predictable possibility of neighboring slices. At the core of our method is a simple and straightforward dense self-supervision on the predictions of local representations and a strategy of predicting locals based on global context, which enables stable and reliable supervision for both global and local representation mining among volumes. Specifically, we first proposed an asymmetric network with an attention-guided predictor to enforce distance-specific prediction and supervision on slices within and across volumes/sequences. Secondly, we introduced a novel prototype-based foreground-background calibration module to enhance representation consistency. The two parts are trained jointly on labeled and unlabeled data. When evaluated on three benchmark datasets of medical volumes and sequences, our model outperforms existing methods with a large margin of 4.5\% DSC on ACDC, 1.7\% on Prostate, and 2.3\% on CAMUS. Intensive evaluations reveals the effectiveness and superiority of our method.

Joint Landmark and Structure Learning for Automatic Evaluation of Developmental Dysplasia of the Hip

The ultrasound (US) screening of the infant hip is vital for the early diagnosis of developmental dysplasia of the hip (DDH). The US diagnosis of DDH refers to measuring alpha and beta angles that quantify hip joint development. These two angles are calculated from key anatomical landmarks and structures of the hip. However, this measurement process is not trivial for sonographers and usually requires a thorough understanding of complex anatomical structures. In this study, we propose a multi-task framework to learn the relationships among landmarks and structures jointly and automatically evaluate DDH. Our multi-task networks are equipped with three novel modules. Firstly, we adopt Mask R-CNN as the basic framework to detect and segment key anatomical structures and add one landmark detection branch to form a new multi-task framework. Secondly, we propose a novel shape similarity loss to refine the incomplete anatomical structure prediction robustly and accurately. Thirdly, we further incorporate the landmark-structure consistent prior to ensure the consistency of the bony rim estimated from the segmented structure and the detected landmark. In our experiments, 1,231 US images of the infant hip from 632 patients are collected, of which 247 images from 126 patients are tested. The average errors in alpha and beta angles are 2.221 degrees and 2.899 degrees. About 93% and 85% estimates of alpha and beta angles have errors less than 5 degrees, respectively. Experimental results demonstrate that the proposed method can accurately and robustly realize the automatic evaluation of DDH, showing great potential for clinical application.

Searching Collaborative Agents for Multi-plane Localization in 3D Ultrasound

3D ultrasound (US) has become prevalent due to its rich spatial and diagnostic information not contained in 2D US. Moreover, 3D US can contain multiple standard planes (SPs) in one shot. Thus, automatically localizing SPs in 3D US has the potential to improve user-independence and scanning-efficiency. However, manual SP localization in 3D US is challenging because of the low image quality, huge search space and large anatomical variability. In this work, we propose a novel multi-agent reinforcement learning (MARL) framework to simultaneously localize multiple SPs in 3D US. Our contribution is four-fold. First, our proposed method is general and it can accurately localize multiple SPs in different challenging US datasets. Second, we equip the MARL system with a recurrent neural network (RNN) based collaborative module, which can strengthen the communication among agents and learn the spatial relationship among planes effectively. Third, we explore to adopt the neural architecture search (NAS) to automatically design the network architecture of both the agents and the collaborative module. Last, we believe we are the first to realize automatic SP localization in pelvic US volumes, and note that our approach can handle both normal and abnormal uterus cases. Extensively validated on two challenging datasets of the uterus and fetal brain, our proposed method achieves the average localization accuracy of 7.03 degrees/1.59mm and 9.75 degrees/1.19mm. Experimental results show that our light-weight MARL model has higher accuracy than state-of-the-art methods.

Learn Fine-grained Adaptive Loss for Multiple Anatomical Landmark Detection in Medical Images

Automatic and accurate detection of anatomical landmarks is an essential operation in medical image analysis with a multitude of applications. Recent deep learning methods have improved results by directly encoding the appearance of the captured anatomy with the likelihood maps (i.e., heatmaps). However, most current solutions overlook another essence of heatmap regression, the objective metric for regressing target heatmaps and rely on hand-crafted heuristics to set the target precision, thus being usually cumbersome and task-specific. In this paper, we propose a novel learning-to-learn framework for landmark detection to optimize the neural network and the target precision simultaneously. The pivot of this work is to leverage the reinforcement learning (RL) framework to search objective metrics for regressing multiple heatmaps dynamically during the training process, thus avoiding setting problem-specific target precision. We also introduce an early-stop strategy for active termination of the RL agent's interaction that adapts the optimal precision for separate targets considering exploration-exploitation tradeoffs. This approach shows better stability in training and improved localization accuracy in inference. Extensive experimental results on two different applications of landmark localization: 1) our in-house prenatal ultrasound (US) dataset and 2) the publicly available dataset of cephalometric X-Ray landmark detection, demonstrate the effectiveness of our proposed method. Our proposed framework is general and shows the potential to improve the efficiency of anatomical landmark detection.

A Deep Attentive Convolutional Neural Network for Automatic Cortical Plate Segmentation in Fetal MRI

Fetal cortical plate segmentation is essential in quantitative analysis of fetal brain maturation and cortical folding. Manual segmentation of the cortical plate, or manual refinement of automatic segmentations is tedious and time-consuming. Automatic segmentation of the cortical plate, on the other hand, is challenged by the relatively low resolution of the reconstructed fetal brain MRI scans compared to the thin structure of the cortical plate, partial voluming, and the wide range of variations in the morphology of the cortical plate as the brain matures during gestation. To reduce the burden of manual refinement of segmentations, we have developed a new and powerful deep learning segmentation method. Our method exploits new deep attentive modules with mixed kernel convolutions within a fully convolutional neural network architecture that utilizes deep supervision and residual connections. We evaluated our method quantitatively based on several performance measures and expert evaluations. Results show that our method outperforms several state-of-the-art deep models for segmentation, as well as a state-of-the-art multi-atlas segmentation technique. We achieved average Dice similarity coefficient of 0.87, average Hausdorff distance of 0.96 mm, and average symmetric surface difference of 0.28 mm on reconstructed fetal brain MRI scans of fetuses scanned in the gestational age range of 16 to 39 weeks. With a computation time of less than 1 minute per fetal brain, our method can facilitate and accelerate large-scale studies on normal and altered fetal brain cortical maturation and folding.

Agent with Warm Start and Adaptive Dynamic Termination for Plane Localization in 3D Ultrasound

Accurate standard plane (SP) localization is the fundamental step for prenatal ultrasound (US) diagnosis. Typically, dozens of US SPs are collected to determine the clinical diagnosis. 2D US has to perform scanning for each SP, which is time-consuming and operator-dependent. While 3D US containing multiple SPs in one shot has the inherent advantages of less user-dependency and more efficiency. Automatically locating SP in 3D US is very challenging due to the huge search space and large fetal posture variations. Our previous study proposed a deep reinforcement learning (RL) framework with an alignment module and active termination to localize SPs in 3D US automatically. However, termination of agent search in RL is important and affects the practical deployment. In this study, we enhance our previous RL framework with a newly designed adaptive dynamic termination to enable an early stop for the agent searching, saving at most 67% inference time, thus boosting the accuracy and efficiency of the RL framework at the same time. Besides, we validate the effectiveness and generalizability of our algorithm extensively on our in-house multi-organ datasets containing 433 fetal brain volumes, 519 fetal abdomen volumes, and 683 uterus volumes. Our approach achieves localization error of 2.52mm/10.26 degrees, 2.48mm/10.39 degrees, 2.02mm/10.48 degrees, 2.00mm/14.57 degrees, 2.61mm/9.71 degrees, 3.09mm/9.58 degrees, 1.49mm/7.54 degrees for the transcerebellar, transventricular, transthalamic planes in fetal brain, abdominal plane in fetal abdomen, and mid-sagittal, transverse and coronal planes in uterus, respectively. Experimental results show that our method is general and has the potential to improve the efficiency and standardization of US scanning.

Generalize Ultrasound Image Segmentation via Instant and Plug & Play Style Transfer

Deep segmentation models that generalize to images with unknown appearance are important for real-world medical image analysis. Retraining models leads to high latency and complex pipelines, which are impractical in clinical settings. The situation becomes more severe for ultrasound image analysis because of their large appearance shifts. In this paper, we propose a novel method for robust segmentation under unknown appearance shifts. Our contribution is three-fold. First, we advance a one-stage plug-and-play solution by embedding hierarchical style transfer units into a segmentation architecture. Our solution can remove appearance shifts and perform segmentation simultaneously. Second, we adopt Dynamic Instance Normalization to conduct precise and dynamic style transfer in a learnable manner, rather than previously fixed style normalization. Third, our solution is fast and lightweight for routine clinical adoption. Given 400*400 image input, our solution only needs an additional 0.2ms and 1.92M FLOPs to handle appearance shifts compared to the baseline pipeline. Extensive experiments are conducted on a large dataset from three vendors demonstrate our proposed method enhances the robustness of deep segmentation models.

Contrastive Rendering for Ultrasound Image Segmentation

Ultrasound (US) image segmentation embraced its significant improvement in deep learning era. However, the lack of sharp boundaries in US images still remains an inherent challenge for segmentation. Previous methods often resort to global context, multi-scale cues or auxiliary guidance to estimate the boundaries. It is hard for these methods to approach pixel-level learning for fine-grained boundary generating. In this paper, we propose a novel and effective framework to improve boundary estimation in US images. Our work has three highlights. First, we propose to formulate the boundary estimation as a rendering task, which can recognize ambiguous points (pixels/voxels) and calibrate the boundary prediction via enriched feature representation learning. Second, we introduce point-wise contrastive learning to enhance the similarity of points from the same class and contrastively decrease the similarity of points from different classes. Boundary ambiguities are therefore further addressed. Third, both rendering and contrastive learning tasks contribute to consistent improvement while reducing network parameters. As a proof-of-concept, we performed validation experiments on a challenging dataset of 86 ovarian US volumes. Results show that our proposed method outperforms state-of-the-art methods and has the potential to be used in clinical practice.

Style-invariant Cardiac Image Segmentation with Test-time Augmentation

Deep models often suffer from severe performance drop due to the appearance shift in the real clinical setting. Most of the existing learning-based methods rely on images from multiple sites/vendors or even corresponding labels. However, collecting enough unknown data to robustly model segmentation cannot always hold since the complex appearance shift caused by imaging factors in daily application. In this paper, we propose a novel style-invariant method for cardiac image segmentation. Based on the zero-shot style transfer to remove appearance shift and test-time augmentation to explore diverse underlying anatomy, our proposed method is effective in combating the appearance shift. Our contribution is three-fold. First, inspired by the spirit of universal style transfer, we develop a zero-shot stylization for content images to generate stylized images that appearance similarity to the style images. Second, we build up a robust cardiac segmentation model based on the U-Net structure. Our framework mainly consists of two networks during testing: the ST network for removing appearance shift and the segmentation network. Third, we investigate test-time augmentation to explore transformed versions of the stylized image for prediction and the results are merged. Notably, our proposed framework is fully test-time adaptation. Experiment results demonstrate that our methods are promising and generic for generalizing deep segmentation models.

Computer-aided Tumor Diagnosis in Automated Breast Ultrasound using 3D Detection Network

Automated breast ultrasound (ABUS) is a new and promising imaging modality for breast cancer detection and diagnosis, which could provide intuitive 3D information and coronal plane information with great diagnostic value. However, manually screening and diagnosing tumors from ABUS images is very time-consuming and overlooks of abnormalities may happen. In this study, we propose a novel two-stage 3D detection network for locating suspected lesion areas and further classifying lesions as benign or malignant tumors. Specifically, we propose a 3D detection network rather than frequently-used segmentation network to locate lesions in ABUS images, thus our network can make full use of the spatial context information in ABUS images. A novel similarity loss is designed to effectively distinguish lesions from background. Then a classification network is employed to identify the located lesions as benign or malignant. An IoU-balanced classification loss is adopted to improve the correlation between classification and localization task. The efficacy of our network is verified from a collected dataset of 418 patients with 145 benign tumors and 273 malignant tumors. Experiments show our network attains a sensitivity of 97.66% with 1.23 false positives (FPs), and has an area under the curve(AUC) value of 0.8720.

Searching Collaborative Agents for Multi-plane Localization in 3D Ultrasound

3D ultrasound (US) is widely used due to its rich diagnostic information, portability and low cost. Automated standard plane (SP) localization in US volume not only improves efficiency and reduces user-dependence, but also boosts 3D US interpretation. In this study, we propose a novel Multi-Agent Reinforcement Learning (MARL) framework to localize multiple uterine SPs in 3D US simultaneously. Our contribution is two-fold. First, we equip the MARL with a one-shot neural architecture search (NAS) module to obtain the optimal agent for each plane. Specifically, Gradient-based search using Differentiable Architecture Sampler (GDAS) is employed to accelerate and stabilize the training process. Second, we propose a novel collaborative strategy to strengthen agents' communication. Our strategy uses recurrent neural network (RNN) to learn the spatial relationship among SPs effectively. Extensively validated on a large dataset, our approach achieves the accuracy of 7.05 degree/2.21mm, 8.62 degree/2.36mm and 5.93 degree/0.89mm for the mid-sagittal, transverse and coronal plane localization, respectively. The proposed MARL framework can significantly increase the plane localization accuracy and reduce the computational cost and model size.

A Global Benchmark of Algorithms for Segmenting Late Gadolinium-Enhanced Cardiac Magnetic Resonance Imaging

Segmentation of cardiac images, particularly late gadolinium-enhanced magnetic resonance imaging (LGE-MRI) widely used for visualizing diseased cardiac structures, is a crucial first step for clinical diagnosis and treatment. However, direct segmentation of LGE-MRIs is challenging due to its attenuated contrast. Since most clinical studies have relied on manual and labor-intensive approaches, automatic methods are of high interest, particularly optimized machine learning approaches. To address this, we organized the "2018 Left Atrium Segmentation Challenge" using 154 3D LGE-MRIs, currently the world's largest cardiac LGE-MRI dataset, and associated labels of the left atrium segmented by three medical experts, ultimately attracting the participation of 27 international teams. In this paper, extensive analysis of the submitted algorithms using technical and biological metrics was performed by undergoing subgroup analysis and conducting hyper-parameter analysis, offering an overall picture of the major design choices of convolutional neural networks (CNNs) and practical considerations for achieving state-of-the-art left atrium segmentation. Results show the top method achieved a dice score of 93.2% and a mean surface to a surface distance of 0.7 mm, significantly outperforming prior state-of-the-art. Particularly, our analysis demonstrated that double, sequentially used CNNs, in which a first CNN is used for automatic region-of-interest localization and a subsequent CNN is used for refined regional segmentation, achieved far superior results than traditional methods and pipelines containing single CNNs. This large-scale benchmarking study makes a significant step towards much-improved segmentation methods for cardiac LGE-MRIs, and will serve as an important benchmark for evaluating and comparing the future works in the field.
