The cognitive, affective, and behavioral expression of self-stigma among people who use drugs in online substance use communities

Objectives: To develop a codebook for self-stigma across cognitive, affective, and behavioral domains, and to estimate the prevalence, co-occurrence, and temporal patterns of these indicators in Reddit posts by people who use drugs. Methods: We developed a ten-indicator codebook through consensus-based abductive coding spanning cognitive (self-labeling, pessimism/self-defeatism, deservingness/worthlessness), affective (shame, guilt/self-blame, despair/hopelessness), and behavioral (concealment, anticipated rejection, desire to quit, ambivalence) domains; two coders reached substantial agreement (Cohen's k = 0.72). We then scaled classification with a large language model validated against expert coding (k = 0.73, F1 = 0.80), analyzing 72,115 thread-initiating posts from 1,660 English-language users (2006-2025). Results: 3,838 posts (5.3%) from 1,228 users (74.0%) contained self-stigma; all ten indicators discriminated self-stigma posts (RR 3.6 to 86.2), led by self-labeling (56.0%) and despair/hopelessness (48.5%). Self-stigma was integrated: core and behavioral indicators were strongly associated at the user level (OR = 4.65, 95% CI 3.12-6.94, p < 0.001), and 87.0% of posts with behavioral indicators also contained a core indicator. Contrary to progressive models, behavioral indicators emerged earlier than core ones (desire to quit at median position 0.08 vs. shame at 0.38). Nine of ten indicators were stable across posting trajectories; only pessimism increased (OR = 1.62, 95% CI 1.25-2.10). Conclusion: Among people who use drugs online, self-stigma is an integrated phenomenon in which behavioral indicators rarely appear without internalized ones and often precede them. Most expressions remain stable over time, but pessimism about change deepens, marking a target for early digital intervention and showing that progressive stage models do not map directly onto textual disclosure.

Self-Stigma Is Not a Monolith, but Generic Empathy Is: Persona-Conditioned LLM Support for People Who Use Drugs

Self-stigma predicts treatment avoidance and disengagement among people who use drugs (PWUD), yet conversational systems aiming to provide support typically treat self-stigma expression as a uniform signal. We present a three-phase, proof-of-concept study of a persona-aware approach to LLM support. Latent Profile Analysis (LPA) on indicator-level features from 1,174 self-stigma expressors on Reddit yields a four-persona typology validated against held-out behavioral and linguistic features. Sequential Bayesian and recurrent neural classifiers recover these personas from limited posting histories, substantially outperforming batch and few-shot LLM baselines (macro-F1 = 0.74 at 30 posts). Evaluation by eight clinical experts across three contemporary LLMs revealed a misalignment: persona-matched responses successfully achieved targeted behavioral shifts, yet raters holistically preferred the generic empathy of the persona-neutral baseline. Our findings suggest that holistic empathy judgments and clinically-aligned response design can pull in opposite directions, and that evaluating LLM-based stigma support requires rubrics capable of decomposing the two.

From #Dr00gtiktok to #harmreduction: Exploring Substance Use Hashtags on TikTok

TikTok has emerged as a major source of information and social interaction for youth, raising urgent questions about how substance use discourse manifests and circulates on the platform. This paper presents the first comprehensive analysis of publicly visible, algorithmically surfaced substance-related content on TikTok, drawing on hashtags spanning all major substance categories. Using a mixed-methods approach that combines social network analysis with qualitative content coding, we examined 2,333 substance-related hashtags, identifying 16 distinct hashtag communities and characterizing their structural and thematic relationships. Our network analysis reveals a highly interconnected small-world structure in which recovery-focused hashtags such as \textit{\#addiction}, \textit{\#recovery}, and \textit{\#sober} serve as central bridges between communities. Qualitative analysis of 351 representative videos shows that Recovery Advocacy content (33.9\%) and Satirical content (28.2\%) dominate, while direct substance depiction appears in only 26\% of videos, with active use shown in just 6.5\% of them. These findings suggest that the algorithmically surfaced layer of substance-related discourse on TikTok is predominantly oriented toward recovery, support, and coping rather than explicit promotion of substance use. We further show that hashtag communities and video content are closely aligned, indicating that substance-related discourse on TikTok is shaped through organic community formation within platform affordances rather than widespread adversarial evasion of moderation. This work contributes to social computing research by showing how algorithmic visibility on TikTok shapes the organization of substance-related discourse and the formation of recovery and support communities.

Not a Swiss Army Knife: Academics' Perceptions of Trade-Offs Around Generative Artificial Intelligence Use

In the rapidly evolving landscape of computing disciplines, substantial efforts are being dedicated to unraveling the sociotechnical implications of generative AI (Gen AI). While existing research has manifested in various forms, there remains a notable gap concerning the direct engagement of knowledge workers in academia with Gen AI. We interviewed 17 knowledge workers, including faculty and students, to investigate the social and technical dimensions of Gen AI from their perspective. Our participants raised concerns about the opacity of the data used to train Gen AI. This lack of transparency makes it difficult to identify and address inaccurate, biased, and potentially harmful, information generated by these models. Knowledge workers also expressed worries about Gen AI undermining trust in the relationship between instructor and student and discussed potential solutions, such as pedagogy readiness, to mitigate them. Additionally, participants recognized Gen AI's potential to democratize knowledge by accelerating the learning process and act as an accessible research assistant. However, there were also concerns about potential social and power imbalances stemming from unequal access to such technologies. Our study offers insights into the concerns and hopes of knowledge workers about the ethical use of Gen AI in educational settings and beyond, with implications for navigating this new landscape.

Words Matter: Reducing Stigma in Online Conversations about Substance Use with Large Language Models

Stigma is a barrier to treatment for individuals struggling with substance use disorders (SUD), which leads to significantly lower treatment engagement rates. With only 7% of those affected receiving any form of help, societal stigma not only discourages individuals with SUD from seeking help but isolates them, hindering their recovery journey and perpetuating a cycle of shame and self-doubt. This study investigates how stigma manifests on social media, particularly Reddit, where anonymity can exacerbate discriminatory behaviors. We analyzed over 1.2 million posts, identifying 3,207 that exhibited stigmatizing language towards people who use substances (PWUS). Using Informed and Stylized LLMs, we develop a model for de-stigmatization of these expressions into empathetic language, resulting in 1,649 reformed phrase pairs. Our paper contributes to the field by proposing a computational framework for analyzing stigma and destigmatizing online content, and delving into the linguistic features that propagate stigma towards PWUS. Our work not only enhances understanding of stigma's manifestations online but also provides practical tools for fostering a more supportive digital environment for those affected by SUD. Decoding the Narratives: Analyzing Personal Drug Experiences Shared on Reddit

Online communities such as drug-related subreddits serve as safe spaces for people who use drugs (PWUD), fostering discussions on substance use experiences, harm reduction, and addiction recovery. Users' shared narratives on these forums provide insights into the likelihood of developing a substance use disorder (SUD) and recovery potential. Our study aims to develop a multi-level, multi-label classification model to analyze online user-generated texts about substance use experiences. For this purpose, we first introduce a novel taxonomy to assess the nature of posts, including their intended connections (Inquisition or Disclosure), subjects (e.g., Recovery, Dependency), and specific objectives (e.g., Relapse, Quality, Safety). Using various multi-label classification algorithms on a set of annotated data, we show that GPT-4, when prompted with instructions, definitions, and examples, outperformed all other models. We apply this model to label an additional 1,000 posts and analyze the categories of linguistic expression used within posts in each class. Our analysis shows that topics such as Safety, Combination of Substances, and Mental Health see more disclosure, while discussions about physiological Effects focus on harm reduction. Our work enriches the understanding of PWUD's experiences and informs the broader knowledge base on SUD and drug use.

The Evolution of Substance Use Coverage in the Philadelphia Inquirer

The media's representation of illicit substance use can lead to harmful stereotypes and stigmatization for individuals struggling with addiction, ultimately influencing public perception, policy, and public health outcomes. To explore how the discourse and coverage of illicit drug use changed over time, this study analyzes 157,476 articles published in the Philadelphia Inquirer over a decade. Specifically, the study focuses on articles that mentioned at least one commonly abused substance, resulting in a sample of 3,903 articles. Our analysis shows that cannabis and narcotics are the most frequently discussed classes of drugs. Hallucinogenic drugs are portrayed more positively than other categories, whereas narcotics are portrayed the most negatively. Our research aims to highlight the need for accurate and inclusive portrayals of substance use and addiction in the media.
