Causal Fairness Assessment of Treatment Allocation with Electronic Health Records

Healthcare continues to grapple with the persistent issue of treatment disparities, sparking concerns regarding the equitable allocation of treatments in clinical practice. While various fairness metrics have emerged to assess fairness in decision-making processes, a growing focus has been on causality-based fairness concepts due to their capacity to mitigate confounding effects and reason about bias. However, the application of causal fairness notions in evaluating the fairness of clinical decision-making with electronic health record (EHR) data remains an understudied domain. This study aims to address the methodological gap in assessing causal fairness of treatment allocation with electronic health records data. We propose a causal fairness algorithm to assess fairness in clinical decision-making. Our algorithm accounts for the heterogeneity of patient populations and identifies potential unfairness in treatment allocation by conditioning on patients who have the same likelihood to benefit from the treatment. We apply this framework to a patient cohort with coronary artery disease derived from an EHR database to evaluate the fairness of treatment decisions. In addition, we investigate the impact of social determinants of health on the assessment of causal fairness of treatment allocation.

Assisted Living in the United States: an Open Dataset

An assisted living facility (ALF) is a place where someone can live, have access to social supports such as transportation, and receive assistance with the activities of daily living such as toileting and dressing. Despite the important role of ALFs, they are not required to be certified with Medicare and there is no public national database of these facilities. We present the first public dataset of ALFs in the United States, covering all 50 states and DC with 44,638 facilities and over 1.2 million beds. This dataset can help provide answers to existing public health questions as well as help those in need find a facility. The dataset was validated by replicating the results of a nationwide study of ALFs that uses closed data [4], where the prevalence of ALFs is assessed with respect to county-level socioeconomic variables related to health disparity such as race, disability, and income. To showcase the value of this dataset, we also propose a novel metric to assess access to community-based care. We calculate the average distance an individual in need must travel in order to reach an ALF. The dataset and all relevant code are available at github.com/antonstengel/assisted-living-data.

Longitudinal Analysis of Discussion Topics in an Online Breast Cancer Community using Convolutional Neural Networks

Identifying topics of discussions in online health communities (OHC) is critical to various applications, but can be difficult because topics of OHC content are usually heterogeneous and domain-dependent. In this paper, we provide a multi-class schema, an annotated dataset, and supervised classifiers based on convolutional neural network (CNN) and other models for the task of classifying discussion topics. We apply the CNN classifier to the most popular breast cancer online community, and carry out a longitudinal analysis to show topic distributions and topic changes throughout members' participation. Our experimental results suggest that CNN outperforms other classifiers in the task of topic classification, and that certain trajectories can be detected with respect to topic changes.
