Large Language Model Few-Shot Prompting with Dilemma Training Outperforms Human Surrogates in Predicting Patient Preferences

In serious illness, human surrogates often struggle to accurately predict patient preferences (68% accuracy), causing decision conflict. Personalized Patient Preference Predictor (P4) agents offer a potential solution, but prior prototypes treat values as static ratings, ignoring the contextual, situation-dependent nature of medical choices. Grounded in the 'logic of care', we present P4-DT (Dilemma Training), a P4 agent that constructs a patient decision policy by engaging users with varied medical dilemmas, eliciting individual preference reasoning through bi-directional training. In a study with 12 patient-surrogate dyads, P4-DT predicted patient treatment choices with 81.7% accuracy, significantly exceeding chance (OR = 5.61 [2.03, 15.51], p < .001) and outperforming both unassisted surrogates (55.0%; OR = 3.67 [1.59, 8.47], p = .002) and surrogates assisted by P4-DT (61.7%). Comparative prompt analyses showed that incorporating contextual scenario decisions and open-ended text improved accuracy by 15.0 percentage points over initial values ratings alone. We discuss implications for further testing and designing of context-aware AI agents that embody richer human experience to partner in complex decision-making.

Acts of Configuration: Rethinking Provenance, Temporality and Legitimacy in Post-Mortem Agents

Work on persona-persistent post-mortem agents typically frames design around a life/death binary. This framing neglects a consequential yet under-theorised condition: when individuals remain alive but have impaired decisional capacity. Drawing on a multi-phase workshop in which participants trained and reflected on an AI agent for Advance Care Planning, we examined how people reason about agentic delegation post-capacity loss. Initially, participants favoured bounded agents grounded in first-party authorship and representational fidelity over autonomous or evolving stand-ins. However, temporality introduced novel ideas like adjacent use driven by persona persistence over functional expansion: agents should evolve while users retain capacity, remain static once capacity is lost, but somehow inform adjacent post-mortem uses. We discuss the implications of these findings and propose that the configuration of agents for post-capacity use reshapes our understanding of provenance, temporality, and legitimacy for post-mortem agents.

Words to Describe What I'm Feeling: Exploring the Potential of AI Agents for High Subjectivity Decisions in Advance Care Planning

Loss of decisional capacity, coupled with the increasing absence of reliable human proxies, raises urgent questions about how individuals' values can be represented in Advance Care Planning (ACP). To probe this fraught design space of high-risk, high-subjectivity decision support, we built an experience prototype (\acpagent{}) and asked 15 participants in 4 workshops to train it to be their personal ACP proxy. We analysed their coping strategies and feature requests and mapped the results onto axes of agent autonomy and human control. Our findings show a surprising 86.7\% agreement with \acpagent{}, arguing for a potential new role of AI in ACP where agents act as personal advocates for individuals, building mutual intelligibility over time. We propose that the key areas of future risk that must be addressed are the moderation of users' expectations and designing accountability and oversight over agent deployment and cutoffs.

'SSL?! What on earth is that?': Towards Designing Age-Inclusive Secure Smartphone Browsing

Owing to the increase in 'certified' phishing websites, there is a steady increase in the number of phishing cases and general susceptibility to phishing. Trust mechanisms (e.g., HTTPS Lock Indicators, SSL Certificates) that help differentiate genuine and phishing websites should therefore be evaluated for their effectiveness in preventing vulnerable users from accessing phishing websites. In this article, we present a study involving 18 adults (male-6; female-12) and 12 older adults (male-4; female-8) to understand the usability of current trust mechanisms and preferred modalities in a conceptualized mechanism. In the first part of the study, using Chrome browser on Android, we asked the participants to browse a banking website and a government website for digital particulars. We asked them to identify which one of the two was a phishing website, rate the usability of both websites and provide qualitative feedback on the trust mechanisms. In the second part, we conceptualized an alternative trust mechanism, which allows seeking social, community and AI-based support to make website trust-related decisions. Herein, we asked the participants as to which modality (social, community or AI) they prefer to seek support from and why it is preferred. Using the current trust mechanisms, none of the participants were able to identify the phishing website. As the participants rated the current mechanisms poorly in terms of usability, they expressed various difficulties that largely did not differ between adults and older adults. In the conceptualized mechanism, we observed a notable difference in the preferred modalities, in that, older adults primarily preferred social support. In addition to these overall findings, specific observations suggest that future trust mechanisms should not only consider age-specific needs but also incorporate substantial improvement in terms of usability.

Does Mode of Digital Contact Tracing Affect User Willingness to Share Information? A Quantitative Study

Digital contact tracing can limit the spread of infectious diseases. Nevertheless, there remain barriers to attaining sufficient adoption. In this study, we investigate how willingness to participate in contact tracing is affected by two critical factors: the modes of data collection and the type of data collected. We conducted a scenario-based survey study among 220 respondents in the United States (U.S.) to understand their perceptions about contact tracing associated with automated and manual contact tracing methods. The findings indicate a promising use of smartphones and a combination of public health officials and medical health records as information sources. Through a quantitative analysis, we describe how different modalities and individual demographic factors may affect user compliance in providing four key pieces of information to contact tracing.

From 996 to 007: Challenges of Working from Home During the Epidemic in China

During the COVID-19 epidemic in China, millions of workers in tech companies had to start working from home (WFH). The change was sudden, unexpected and companies were not ready for it. Additionally, it was also the first time that WFH was experienced on such a large scale. We used the opportunity to describe the effect of WFH at scale for a sustained period of time. As the lockdown was easing, we conducted semi-structured interviews with 12 participants from China working in tech companies. While at first, WFH was reported as a pleasant experience with advantages, e.g. flexible schedule, more time with family, over time, this evolved into a rather negative experience where workers start working all day, every day and feel a higher workload despite the actual workload being reduced. We discuss these results and how they could apply for other extreme circumstances and to help improve WFH in general.

AVEID: Automatic Video System for Measuring Engagement In Dementia

Engagement in dementia is typically measured using behavior observational scales (BOS) that are tedious and involve intensive manual labor to annotate, and are therefore not easily scalable. We propose AVEID, a low cost and easy-to-use video-based engagement measurement tool to determine the engagement level of a person with dementia (PwD) during digital interaction. We show that the objective behavioral measures computed via AVEID correlate well with subjective expert impressions for the popular MPES and OME BOS, confirming its viability and effectiveness. Moreover, AVEID measures can be obtained for a variety of engagement designs, thereby facilitating large-scale studies with PwD populations.
