Ground Truths in Suicide Research: The Current State of AI-Based Suicide Detection in Social Media

Recent advances in artificial intelligence (AI) and social media data have led to growing optimism about the ability to detect suicide risk at scale. However, the empirical foundations of this work remain unclear. This article provides a synthesis of current research on AI-based suicide detection in social media, drawing on a recent umbrella review of 22 systematic reviews covering studies up to 2022, alongside an ongoing literature review extending the analysis to more recent work. Across these sources, we identified 195 relevant studies, which are documented in a detailed supplementary dataset outlining their key characteristics and findings (see Supplementary Information). Analysis of these studies reveals consistent patterns, including rapid growth, concentration on a small number of platforms, reliance on textual and English-language data, and repeated use of similar datasets. Most importantly, the majority of studies rely on indirect labeling strategies that do not involve direct, individual-level validation of suicide risk. Instead, ground truth is typically inferred from observable features of online content, such as linguistic markers or community membership. As a result, the predictive task often shifts from identifying individuals at risk to classifying posts that contain suicidal or distress-related language, limiting the ability of current approaches to detect individuals who do not express such content explicitly online. These findings suggest that current advances in model performance should be interpreted with caution. Progress in this field is likely to depend less on improving model performance and more on ensuring that model predictions meaningfully correspond to suicide risk as it is experienced in real life.

Artificial intelligence is creating a new global linguistic hierarchy

Artificial intelligence (AI) has the potential to transform healthcare, education, governance and socioeconomic equity, but its benefits remain concentrated in a small number of languages (Bender, 2019; Blasi et al., 2022; Joshi et al., 2020; Ranathunga and de Silva, 2022; Young, 2015). Language AI - the technologies that underpin widely-used conversational systems such as ChatGPT - could provide major benefits if available in people's native languages, yet most of the world's 7,000+ linguistic communities currently lack access and face persistent digital marginalization. Here we present a global longitudinal analysis of social, economic and infrastructural conditions across languages to assess systemic inequalities in language AI. We first analyze the existence of AI resources for 6003 languages. We find that despite efforts of the community to broaden the reach of language technologies (Bapna et al., 2022; Costa-Juss\`a et al., 2022), the dominance of a handful of languages is exacerbating disparities on an unprecedented scale, with divides widening exponentially rather than narrowing. Further, we contrast the longitudinal diffusion of AI with that of earlier IT technologies, revealing a distinctive hype-driven pattern of spread. To translate our findings into practical insights and guide prioritization efforts, we introduce the Language AI Readiness Index (EQUATE), which maps the state of technological, socio-economic, and infrastructural prerequisites for AI deployment across languages. The index highlights communities where capacity exists but remains underutilized, and provides a framework for accelerating more equitable diffusion of language AI. Our work contributes to setting the baseline for a transition towards more sustainable and equitable language technologies.

Bridging Online Behavior and Clinical Insight: A Longitudinal LLM-based Study of Suicidality on YouTube Reveals Novel Digital Markers

Suicide remains a leading cause of death in Western countries. As social media becomes central to daily life, digital footprints offer valuable insight into suicidal behavior. Focusing on individuals who attempted suicide while uploading videos to their channels, we investigate: How do linguistic patterns on YouTube reflect suicidal behavior, and how do these patterns align with or differ from expert knowledge? We examined linguistic changes around suicide attempts and compared individuals who attempted suicide while actively uploading to their channel with three control groups: those with prior attempts, those experiencing major life events, and matched individuals from the broader cohort. Applying complementary bottom-up, hybrid, and expert-driven approaches, we analyzed a novel longitudinal dataset of 181 suicide-attempt channels and 134 controls. In the bottom-up analysis, LLM-based topic-modeling identified 166 topics; five were linked to suicide attempts, two also showed attempt-related temporal changes (Mental Health Struggles, $OR = 1.74$; YouTube Engagement, $OR = 1.67$; $p < .01$). In the hybrid approach, clinical experts reviewed LLM-derived topics and flagged 19 as suicide-related. However, none showed significant effects beyond those identified bottom-up. YouTube Engagement, a platform-specific indicator, was not flagged, underscoring the value of bottom-up discovery. A top-down psychological assessment of suicide narratives revealed differing motivations: individuals describing prior attempts aimed to help others ($\beta=-1.69$, $p<.01$), whereas those attempted during the uploading period emphasized personal recovery ($\beta=1.08$, $p<.01$). By integrating these approaches, we offer a nuanced understanding of suicidality, bridging digital behavior and clinical insights.

Bored to Death: Artificial Intelligence Research Reveals the Role of Boredom in Suicide Behavior

Background: Recent advancements in Artificial Intelligence (AI) contributed significantly to suicide assessment, however, our theoretical understanding of this complex behavior is still limited. Objective: This study aimed to harness AI methodologies to uncover hidden risk factors that trigger or aggravate suicide behaviors. Method: The primary dataset included 228,052 Facebook postings by 1,006 users who completed the gold-standard Columbia Suicide Severity Rating Scale. This dataset was analyzed using a bottom-up research pipeline without a-priory hypotheses and its findings were validated using a top-down analysis of a new dataset. This secondary dataset included responses by 1,062 participants to the same suicide scale as well as to well-validated scales measuring depression and boredom. Results: An almost fully automated, AI-guided research pipeline resulted in four Facebook topics that predicted the risk of suicide, of which the strongest predictor was boredom. A comprehensive literature review using APA PsycInfo revealed that boredom is rarely perceived as a unique risk factor of suicide. A complementing top-down path analysis of the secondary dataset uncovered an indirect relationship between boredom and suicide, which was mediated by depression. An equivalent mediated relationship was observed in the primary Facebook dataset as well. However, here, a direct relationship between boredom and suicide risk was also observed. Conclusions: Integrating AI methods allowed the discovery of an under-researched risk factor of suicide. The study signals boredom as a maladaptive 'ingredient' that might trigger suicide behaviors, regardless of depression. Further studies are recommended to direct clinicians' attention to this burdening, and sometimes existential experience.

A Picture May Be Worth a Thousand Lives: An Interpretable Artificial Intelligence Strategy for Predictions of Suicide Risk from Social Media Images

The promising research on Artificial Intelligence usages in suicide prevention has principal gaps, including black box methodologies, inadequate outcome measures, and scarce research on non-verbal inputs, such as social media images (despite their popularity today, in our digital era). This study addresses these gaps and combines theory-driven and bottom-up strategies to construct a hybrid and interpretable prediction model of valid suicide risk from images. The lead hypothesis was that images contain valuable information about emotions and interpersonal relationships, two central concepts in suicide-related treatments and theories. The dataset included 177,220 images by 841 Facebook users who completed a gold-standard suicide scale. The images were represented with CLIP, a state-of-the-art algorithm, which was utilized, unconventionally, to extract predefined features that served as inputs to a simple logistic-regression prediction model (in contrast to complex neural networks). The features addressed basic and theory-driven visual elements using everyday language (e.g., bright photo, photo of sad people). The results of the hybrid model (that integrated theory-driven and bottom-up methods) indicated high prediction performance that surpassed common bottom-up algorithms, thus providing a first proof that images (alone) can be leveraged to predict validated suicide risk. Corresponding with the lead hypothesis, at-risk users had images with increased negative emotions and decreased belonginess. The results are discussed in the context of non-verbal warning signs of suicide. Notably, the study illustrates the advantages of hybrid models in such complicated tasks and provides simple and flexible prediction strategies that could be utilized to develop real-life monitoring tools of suicide.
