Multimodal risk trajectories reveal heterogeneous paths to dementia

Dementia comprises biologically heterogeneous disorders, yet current risk assessment provides limited insight into how subtype-specific risk emerges and diverges before clinical diagnosis. We developed NetMoint, a multimodal framework integrating partially observed plasma proteomic, structural magnetic resonance imaging and cerebral haemodynamic phenotypes to predict individualized risks of Alzheimer's disease (AD), vascular dementia (VD) and frontotemporal dementia (FTD) across 1-, 5-, 10- and 20-year horizons. Among 104,120 UK Biobank participants free of dementia at baseline, NetMoint achieved mean area under the receiver operating characteristic curve (AUC) values of 0.937, 0.930 and 0.932 for AD, VD and FTD, respectively. The biological determinants of prediction shifted with time, from structural brain vulnerability at shorter horizons towards circulating molecular signatures at longer horizons, with distinct subtype-specific biological profiles. Multi-horizon risk profiling identified distinct temporal trajectories of dementia susceptibility. Among participants who subsequently developed AD, 0.7% followed a persistently very-high-risk trajectory, with predicted risk reaching 53.50% at 20 years, whereas 8.3% of those who developed FTD followed an increasing very-high-risk trajectory, reaching 67.17%. These high-risk trajectories were marked by distinct molecular signatures, with lower TGFB1 characterizing the AD group and higher NDRG1 the FTD group. In an independent ADNI-to-UK Biobank analysis, AD risk prediction remained informative after harmonization to 138 shared features, with an AUC of 0.741 at 20 years. Together, these findings establish a multimodal framework for trajectory-resolved dementia risk stratification, identifying small but high-risk populations within dementia subtypes and linking their divergent risk trajectories to distinct molecular signatures.

CoMoL: Efficient Mixture of LoRA Experts via Dynamic Core Space Merging

Large language models (LLMs) achieve remarkable performance on diverse downstream and domain-specific tasks via parameter-efficient fine-tuning (PEFT). However, existing PEFT methods, particularly MoE-LoRA architectures, suffer from limited parameter efficiency and coarse-grained adaptation due to the proliferation of LoRA experts and instance-level routing. To address these issues, we propose Core Space Mixture of LoRA (\textbf{CoMoL}), a novel MoE-LoRA framework that incorporates expert diversity, parameter efficiency, and fine-grained adaptation. Specifically, CoMoL introduces two key components: core space experts and core space routing. Core space experts store each expert in a compact core matrix, preserving diversity while controlling parameter growth. Core space routing dynamically selects and activates the appropriate core experts for each token, enabling fine-grained, input-adaptive routing. Activated core experts are then merged via a soft-merging strategy into a single core expert, which is combined with a shared LoRA to form a specialized LoRA module. Besides, the routing network is projected into the same low-rank space as the LoRA matrices, further reducing parameter overhead without compromising expressiveness. Extensive experiments demonstrate that CoMoL retains the adaptability of MoE-LoRA architectures while achieving parameter efficiency comparable to standard LoRA, consistently outperforming existing methods across multiple tasks.

IAD-Unify: A Region-Grounded Unified Model for Industrial Anomaly Segmentation, Understanding, and Generation

Real-world industrial inspection requires not only localizing defects, but also explaining them in natural language and generating controlled defect edits. However, existing approaches fail to jointly support all three capabilities within a unified framework and evaluation protocol. We propose IAD-Unify, a dual-encoder unified framework in which a frozen DINOv2-based region expert supplies precise anomaly evidence to a shared Qwen3.5-4B vision-language backbone via lightweight token injection, jointly enabling anomaly segmentation, region-grounded understanding, and mask-guided generation. To enable unified evaluation, we further construct Anomaly-56K, a comprehensive unified multi-task IAD evaluation platform, spanning 59,916 images across 24 categories and 104 defect variants. Controlled ablations yield four findings: (i) region grounding is the decisive mechanism for understanding, removing it degrades location accuracy by >76 pp; (ii) predicted-region performance closely matches oracle, confirming deployment viability; (iii) region-grounded generation achieves the best full-image fidelity and masked-region perceptual quality; and (iv) pre-initialized joint training improves understanding at negligible generation cost (-0.16 dB). IAD-Unify further achieves strong performance on the MMAD benchmark, including categories unseen during training, demonstrating robust cross-category generalization.

HeartcareGPT: A Unified Multimodal ECG Suite for Dual Signal-Image Modeling and Understanding

Although electrocardiograms (ECG) play a dominant role in cardiovascular diagnosis and treatment, their intrinsic data forms and representational patterns pose significant challenges for medical multimodal large language models (Med-MLLMs) in achieving cross-modal semantic alignment. To address this gap, we propose Heartcare Suite, a unified ECG suite designed for dual signal-image modeling and understanding: (i) Heartcare-400K. A fine-grained ECG instruction dataset on top of our data pipeline engine--HeartAgent--by integrating high quality clinical ECG reports from top hospitals with open-source data. (ii) Heartcare-Bench. A systematic benchmark assessing performance of models in multi-perspective ECG understanding and cross-modal generalization, providing guidance for optimizing ECG comprehension models. (iii) HeartcareGPT. Built upon a structure-aware discrete tokenizer Beat, we propose Dual Stream Projection Alignment (DSPA) paradigm--a dual encoder projection alignment mechanism enabling joint optimizing and modeling native ECG signal-image within a shared feature space. HeartcareGPT achieves consistent improvements across diverse ECG understanding tasks, validating both the effectiveness of the unified modeling paradigm and the necessity of a high-quality data pipeline, and establishing a methodological foundation for extending Med-MLLMs towards physiological signal domains. CLEAR-Mamba:Towards Accurate, Adaptive and Trustworthy Multi-Sequence Ophthalmic Angiography Classification

Medical image classification is a core task in computer-aided diagnosis (CAD), playing a pivotal role in early disease detection, treatment planning, and patient prognosis assessment. In ophthalmic practice, fluorescein fundus angiography (FFA) and indocyanine green angiography (ICGA) provide hemodynamic and lesion-structural information that conventional fundus photography cannot capture. However, due to the single-modality nature, subtle lesion patterns, and significant inter-device variability, existing methods still face limitations in generalization and high-confidence prediction. To address these challenges, we propose CLEAR-Mamba, an enhanced framework built upon MedMamba with optimizations in both architecture and training strategy. Architecturally, we introduce HaC, a hypernetwork-based adaptive conditioning layer that dynamically generates parameters according to input feature distributions, thereby improving cross-domain adaptability. From a training perspective, we develop RaP, a reliability-aware prediction scheme built upon evidential uncertainty learning, which encourages the model to emphasize low-confidence samples and improves overall stability and reliability. We further construct a large-scale ophthalmic angiography dataset covering both FFA and ICGA modalities, comprising multiple retinal disease categories for model training and evaluation. Experimental results demonstrate that CLEAR-Mamba consistently outperforms multiple baseline models, including the original MedMamba, across various metrics-showing particular advantages in multi-disease classification and reliability-aware prediction. This study provides an effective solution that balances generalizability and reliability for modality-specific medical image classification tasks. MAU-GPT: Enhancing Multi-type Industrial Anomaly Understanding via Anomaly-aware and Generalist Experts Adaptation

As industrial manufacturing scales, automating fine-grained product image analysis has become critical for quality control. However, existing approaches are hindered by limited dataset coverage and poor model generalization across diverse and complex anomaly patterns. To address these challenges, we introduce MAU-Set, a comprehensive dataset for Multi-type industrial Anomaly Understanding. It spans multiple industrial domains and features a hierarchical task structure, ranging from binary classification to complex reasoning. Alongside this dataset, we establish a rigorous evaluation protocol to facilitate fair and comprehensive model assessment. Building upon this foundation, we further present MAU-GPT, a domain-adapted multimodal large model specifically designed for industrial anomaly understanding. It incorporates a novel AMoE-LoRA mechanism that unifies anomaly-aware and generalist experts adaptation, enhancing both understanding and reasoning across diverse defect classes. Extensive experiments show that MAU-GPT consistently outperforms prior state-of-the-art methods across all domains, demonstrating strong potential for scalable and automated industrial inspection.

Fast Thinking for Large Language Models

Reasoning-oriented Large Language Models (LLMs) often rely on generating explicit tokens step by step, and their effectiveness typically hinges on large-scale supervised fine-tuning or reinforcement learning. While Chain-of-Thought (CoT) techniques substantially enhance performance on complex reasoning tasks, they remain inefficient, requiring long reasoning traces that increase latency and token usage. In this work, we introduce Latent Codebooks for Fast Thinking, a framework that uses concise CoT sketches only during training to learn a codebook of discrete strategy priors. At inference, the model conditions on a handful of continuous thinking vectors distilled from the codebook in a single pass, enabling strategy-level guidance without producing explicit reasoning tokens. To complement this design, we propose GainRouter, a lightweight routing mechanism that adaptively switches between fast codebook guided inference and slow explicit reasoning, thereby suppressing overthinking and reducing unnecessary token generation. Experiments across multiple reasoning benchmarks show that our approach achieves competitive or superior accuracy while substantially lowering inference cost, offering a practical path toward efficient and controllable reasoning in large language models.
