Beyond N-grams: A Hierarchical Reward Learning Framework for Clinically-Aware Medical Report Generation

Automatic medical report generation can greatly reduce the workload of doctors, but it is often unreliable for real-world deployment. Current methods can write formally fluent sentences but may be factually flawed, introducing serious medical errors known as clinical hallucinations, which make them untrustworthy for diagnosis. To bridge this gap, we introduce HiMed-RL, a Hierarchical Medical Reward Learning Framework designed to explicitly prioritize clinical quality. HiMed-RL moves beyond simple text matching by deconstructing reward learning into three synergistic levels: it first ensures linguistic fluency at the token-level, then enforces factual grounding at the concept-level by aligning key medical terms with expert knowledge, and finally assesses high-level diagnostic consistency at the semantic-level using a specialized LLM verifier. This hierarchical reward is implemented via a Human-inspired Dynamic Reward Adjustment, a strategy which first teaches the model to learn basic facts before progressing to more complex diagnostic reasoning. Experimentally, HiMed-3B achieves state-of-the-art performance on both in-domain and out-of-domain benchmarks, particularly on the latter, with an improvement of 12.1% over the second-best baseline. Our work provides a robust paradigm for generating reports that not only improve fluency but clinical fine-grained quality.

Med-GLIP: Advancing Medical Language-Image Pre-training with Large-scale Grounded Dataset

Medical image grounding aims to align natural language phrases with specific regions in medical images, serving as a foundational task for intelligent diagnosis, visual question answering (VQA), and automated report generation (MRG). However, existing research is constrained by limited modality coverage, coarse-grained annotations, and the absence of a unified, generalizable grounding framework. To address these challenges, we construct a large-scale medical grounding dataset Med-GLIP-5M comprising over 5.3 million region-level annotations across seven imaging modalities, covering diverse anatomical structures and pathological findings. The dataset supports both segmentation and grounding tasks with hierarchical region labels, ranging from organ-level boundaries to fine-grained lesions. Based on this foundation, we propose Med-GLIP, a modality-aware grounding framework trained on Med-GLIP-5M. Rather than relying on explicitly designed expert modules, Med-GLIP implicitly acquires hierarchical semantic understanding from diverse training data -- enabling it to recognize multi-granularity structures, such as distinguishing lungs from pneumonia lesions. Extensive experiments demonstrate that Med-GLIP consistently outperforms state-of-the-art baselines across multiple grounding benchmarks. Furthermore, integrating its spatial outputs into downstream tasks, including medical VQA and report generation, leads to substantial performance gains. Our dataset will be released soon.

Hulu-Med: A Transparent Generalist Model towards Holistic Medical Vision-Language Understanding

Real-world clinical decision-making requires integrating heterogeneous data, including medical text, 2D images, 3D volumes, and videos, while existing AI systems fail to unify all these signals, limiting their utility. In this paper, we introduce Hulu-Med, a transparent, generalist medical Vision-Language Model (VLM) designed to unify language-only, 2D/3D vision-language, and video understanding within a single architecture. Hulu-Med is trained on a curated corpus of 16.7 million samples, comprising exclusively public or synthetic data, spanning 12 major anatomical systems and 14 medical imaging modalities. Hulu-Med employs a medical-aware token-reduction strategy that prunes redundant visual tokens, achieving up to a 55% reduction for 3D and video inputs, improving cross-modal efficiency, and enabling training at 7B-32B parameter scales in approximately 4,000-40,000 GPU hours. Across 30 public in-domain and out-of-domain medical benchmarks-covering text reasoning, visual question answering, report generation, multilingual dialogue, video understanding, and rare disease diagnosis-Hulu-Med surpasses existing open-source models on 27 of 30 benchmarks and outperforms proprietary systems such as GPT-4o on 16 benchmarks. Despite being a VLM, Hulu-Med outperforms GPT-4o and matches GPT-o1 on the text-only HealthBench. For the first time in the community, we provide a fully transparent, reproducible and cost-effective pipeline for holistic medical vision-language understanding by releasing our end-to-end data curation, training procedures, and model parameters. Code and models are available at https://github.com/ZJUI-AI4H/Hulu-Med.

LongWeave: A Long-Form Generation Benchmark Bridging Real-World Relevance and Verifiability

Generating long, informative, and factual outputs remains a major challenge for Large Language Models (LLMs). Existing benchmarks for long-form generation typically assess real-world queries with hard-to-verify metrics or use synthetic setups that ease evaluation but overlook real-world intricacies. In this paper, we introduce \textbf{LongWeave}, which balances real-world and verifiable assessment with Constraint-Verifier Evaluation (CoV-Eval). CoV-Eval constructs tasks by first defining verifiable targets within real-world scenarios, then systematically generating corresponding queries, textual materials, and constraints based on these targets. This ensures that tasks are both realistic and objectively assessable, enabling rigorous assessment of model capabilities in meeting complex real-world constraints. LongWeave supports customizable input/output lengths (up to 64K/8K tokens) across seven distinct tasks. Evaluation on 23 LLMs shows that even state-of-the-art models encounter significant challenges in long-form generation as real-world complexity and output length increase.

Medical Visual Question Answering (Med-VQA) holds significant potential for clinical decision support, yet existing efforts primarily focus on 2D imaging with limited task diversity. This paper presents 3D-RAD, a large-scale dataset designed to advance 3D Med-VQA using radiology CT scans. The 3D-RAD dataset encompasses six diverse VQA tasks: anomaly detection, image observation, medical computation, existence detection, static temporal diagnosis, and longitudinal temporal diagnosis. It supports both open- and closed-ended questions while introducing complex reasoning challenges, including computational tasks and multi-stage temporal analysis, to enable comprehensive benchmarking. Extensive evaluations demonstrate that existing vision-language models (VLMs), especially medical VLMs exhibit limited generalization, particularly in multi-temporal tasks, underscoring the challenges of real-world 3D diagnostic reasoning. To drive future advancements, we release a high-quality training set 3D-RAD-T of 136,195 expert-aligned samples, showing that fine-tuning on this dataset could significantly enhance model performance. Our dataset and code, aiming to catalyze multimodal medical AI research and establish a robust foundation for 3D medical visual understanding, are publicly available at https://github.com/Tang-xiaoxiao/3D-RAD.

Modest-Align: Data-Efficient Alignment for Vision-Language Models

Cross-modal alignment aims to map heterogeneous modalities into a shared latent space, as exemplified by models like CLIP, which benefit from large-scale image-text pretraining for strong recognition capabilities. However, when operating in resource-constrained settings with limited or low-quality data, these models often suffer from overconfidence and degraded performance due to the prevalence of ambiguous or weakly correlated image-text pairs. Current contrastive learning approaches, which rely on single positive pairs, further exacerbate this issue by reinforcing overconfidence on uncertain samples. To address these challenges, we propose Modest-Align, a lightweight alignment framework designed for robustness and efficiency. Our approach leverages two complementary strategies -- Random Perturbation, which introduces controlled noise to simulate uncertainty, and Embedding Smoothing, which calibrates similarity distributions in the embedding space. These mechanisms collectively reduce overconfidence and improve performance on noisy or weakly aligned samples. Extensive experiments across multiple benchmark datasets demonstrate that Modest-Align outperforms state-of-the-art methods in retrieval tasks, achieving competitive results with over 100x less training data and 600x less GPU time than CLIP. Our method offers a practical and scalable solution for cross-modal alignment in real-world, low-resource scenarios.

DentVLM: A Multimodal Vision-Language Model for Comprehensive Dental Diagnosis and Enhanced Clinical Practice

Diagnosing and managing oral diseases necessitate advanced visual interpretation across diverse imaging modalities and integrated information synthesis. While current AI models excel at isolated tasks, they often fall short in addressing the complex, multimodal requirements of comprehensive clinical dental practice. Here we introduce DentVLM, a multimodal vision-language model engineered for expert-level oral disease diagnosis. DentVLM was developed using a comprehensive, large-scale, bilingual dataset of 110,447 images and 2.46 million visual question-answering (VQA) pairs. The model is capable of interpreting seven 2D oral imaging modalities across 36 diagnostic tasks, significantly outperforming leading proprietary and open-source models by 19.6% higher accuracy for oral diseases and 27.9% for malocclusions. In a clinical study involving 25 dentists, evaluating 1,946 patients and encompassing 3,105 QA pairs, DentVLM surpassed the diagnostic performance of 13 junior dentists on 21 of 36 tasks and exceeded that of 12 senior dentists on 12 of 36 tasks. When integrated into a collaborative workflow, DentVLM elevated junior dentists' performance to senior levels and reduced diagnostic time for all practitioners by 15-22%. Furthermore, DentVLM exhibited promising performance across three practical utility scenarios, including home-based dental health management, hospital-based intelligent diagnosis and multi-agent collaborative interaction. These findings establish DentVLM as a robust clinical decision support tool, poised to enhance primary dental care, mitigate provider-patient imbalances, and democratize access to specialized medical expertise within the field of dentistry.

MT-RewardTree: A Comprehensive Framework for Advancing LLM-Based Machine Translation via Reward Modeling

Process reward models (PRMs) have shown success in complex reasoning tasks for large language models (LLMs). However, their application to machine translation (MT) remains underexplored due to the lack of systematic methodologies and evaluation benchmarks. To address this gap, we introduce \textbf{MT-RewardTree}, a comprehensive framework for constructing, evaluating, and deploying process reward models in MT. Unlike traditional vanilla preference pair construction, we propose a novel method for automatically generating token-level preference pairs using approximate Monte Carlo Tree Search (MCTS), which mitigates the prohibitive cost of human annotation for fine-grained steps. Then, we establish the first MT-specific reward model benchmark and provide a systematic comparison of different reward modeling architectures, revealing that token-level supervision effectively captures fine-grained preferences. Experimental results demonstrate that our MT-PRM-Qwen-2.5-3B achieves state-of-the-art performance in both token-level and sequence-level evaluation given the same input prefix. Furthermore, we showcase practical applications where PRMs enable test-time alignment for LLMs without additional alignment training and significantly improve performance in hypothesis ensembling. Our work provides valuable insights into the role of reward models in MT research. Our code and data are released in \href{https://sabijun.github.io/MT_RewardTreePage/}{https://sabijun.github.io/MT\_RewardTreePage}.

DentalBench: Benchmarking and Advancing LLMs Capability for Bilingual Dentistry Understanding

Recent advances in large language models (LLMs) and medical LLMs (Med-LLMs) have demonstrated strong performance on general medical benchmarks. However, their capabilities in specialized medical fields, such as dentistry which require deeper domain-specific knowledge, remain underexplored due to the lack of targeted evaluation resources. In this paper, we introduce DentalBench, the first comprehensive bilingual benchmark designed to evaluate and advance LLMs in the dental domain. DentalBench consists of two main components: DentalQA, an English-Chinese question-answering (QA) benchmark with 36,597 questions spanning 4 tasks and 16 dental subfields; and DentalCorpus, a large-scale, high-quality corpus with 337.35 million tokens curated for dental domain adaptation, supporting both supervised fine-tuning (SFT) and retrieval-augmented generation (RAG). We evaluate 14 LLMs, covering proprietary, open-source, and medical-specific models, and reveal significant performance gaps across task types and languages. Further experiments with Qwen-2.5-3B demonstrate that domain adaptation substantially improves model performance, particularly on knowledge-intensive and terminology-focused tasks, and highlight the importance of domain-specific benchmarks for developing trustworthy and effective LLMs tailored to healthcare applications.

Knowing or Guessing? Robust Medical Visual Question Answering via Joint Consistency and Contrastive Learning

In high-stakes medical applications, consistent answering across diverse question phrasings is essential for reliable diagnosis. However, we reveal that current Medical Vision-Language Models (Med-VLMs) exhibit concerning fragility in Medical Visual Question Answering, as their answers fluctuate significantly when faced with semantically equivalent rephrasings of medical questions. We attribute this to two limitations: (1) insufficient alignment of medical concepts, leading to divergent reasoning patterns, and (2) hidden biases in training data that prioritize syntactic shortcuts over semantic understanding. To address these challenges, we construct RoMed, a dataset built upon original VQA datasets containing 144k questions with variations spanning word-level, sentence-level, and semantic-level perturbations. When evaluating state-of-the-art (SOTA) models like LLaVA-Med on RoMed, we observe alarming performance drops (e.g., a 40\% decline in Recall) compared to original VQA benchmarks, exposing critical robustness gaps. To bridge this gap, we propose Consistency and Contrastive Learning (CCL), which integrates two key components: (1) knowledge-anchored consistency learning, aligning Med-VLMs with medical knowledge rather than shallow feature patterns, and (2) bias-aware contrastive learning, mitigating data-specific priors through discriminative representation refinement. CCL achieves SOTA performance on three popular VQA benchmarks and notably improves answer consistency by 50\% on the challenging RoMed test set, demonstrating significantly enhanced robustness. Code will be released.

Datasets and Recipes for Video Temporal Grounding via Reinforcement Learning

Video Temporal Grounding (VTG) aims to localize relevant temporal segments in videos given natural language queries. Despite recent progress with large vision-language models (LVLMs) and instruction-tuning, existing approaches often suffer from limited temporal awareness and poor generalization. In this work, we introduce a two-stage training framework that integrates supervised fine-tuning with reinforcement learning (RL) to improve both the accuracy and robustness of VTG models. Our approach first leverages high-quality curated cold start data for SFT initialization, followed by difficulty-controlled RL to further enhance temporal localization and reasoning abilities. Comprehensive experiments on multiple VTG benchmarks demonstrate that our method consistently outperforms existing models, particularly in challenging and open-domain scenarios. We conduct an in-depth analysis of training strategies and dataset curation, highlighting the importance of both high-quality cold start data and difficulty-controlled RL. To facilitate further research and industrial adoption, we release all intermediate datasets, models, and code to the community.

ReCamMaster: Camera-Controlled Generative Rendering from A Single Video

Camera control has been actively studied in text or image conditioned video generation tasks. However, altering camera trajectories of a given video remains under-explored, despite its importance in the field of video creation. It is non-trivial due to the extra constraints of maintaining multiple-frame appearance and dynamic synchronization. To address this, we present ReCamMaster, a camera-controlled generative video re-rendering framework that reproduces the dynamic scene of an input video at novel camera trajectories. The core innovation lies in harnessing the generative capabilities of pre-trained text-to-video models through a simple yet powerful video conditioning mechanism--its capability is often overlooked in current research. To overcome the scarcity of qualified training data, we construct a comprehensive multi-camera synchronized video dataset using Unreal Engine 5, which is carefully curated to follow real-world filming characteristics, covering diverse scenes and camera movements. It helps the model generalize to in-the-wild videos. Lastly, we further improve the robustness to diverse inputs through a meticulously designed training strategy. Extensive experiments show that our method substantially outperforms existing state-of-the-art approaches. Our method also finds promising applications in video stabilization, super-resolution, and outpainting. Our code and dataset are publicly available at: https://github.com/KwaiVGI/ReCamMaster.

FairSteer: Inference Time Debiasing for LLMs with Dynamic Activation Steering

Large language models (LLMs) are prone to capturing biases from training corpus, leading to potential negative social impacts. Existing prompt-based debiasing methods exhibit instability due to their sensitivity to prompt changes, while fine-tuning-based techniques incur substantial computational overhead and catastrophic forgetting. In this paper, we propose FairSteer, a novel inference-time debiasing framework without requiring customized prompt design or model retraining. Motivated by the linear representation hypothesis, our preliminary investigation demonstrates that fairness-related features can be encoded into separable directions in the hidden activation space. FairSteer operates in three steps: biased activation detection, debiasing steering vector (DSV) computation, and dynamic activation steering. Specifically, it first trains a lightweight linear classifier to detect bias signatures in activations, and then computes DSVs as intervention directions derived from small contrastive prompt pairs. Subsequently, it performs debiasing by adjusting activations with DSVs in the inference stage. Comprehensive evaluation with six LLMs demonstrates the superiority of FairSteer across question-answering, counterfactual input evaluation and open-ended text generation tasks. Code will be released.

MedEthicsQA: A Comprehensive Question Answering Benchmark for Medical Ethics Evaluation of LLMs

While Medical Large Language Models (MedLLMs) have demonstrated remarkable potential in clinical tasks, their ethical safety remains insufficiently explored. This paper introduces $\textbf{MedEthicsQA}$, a comprehensive benchmark comprising $\textbf{5,623}$ multiple-choice questions and $\textbf{5,351}$ open-ended questions for evaluation of medical ethics in LLMs. We systematically establish a hierarchical taxonomy integrating global medical ethical standards. The benchmark encompasses widely used medical datasets, authoritative question banks, and scenarios derived from PubMed literature. Rigorous quality control involving multi-stage filtering and multi-faceted expert validation ensures the reliability of the dataset with a low error rate ($2.72\%$). Evaluation of state-of-the-art MedLLMs exhibit declined performance in answering medical ethics questions compared to their foundation counterparts, elucidating the deficiencies of medical ethics alignment. The dataset, registered under CC BY-NC 4.0 license, is available at https://github.com/JianhuiWei7/MedEthicsQA.

V2T-CoT: From Vision to Text Chain-of-Thought for Medical Reasoning and Diagnosis

Recent advances in multimodal techniques have led to significant progress in Medical Visual Question Answering (Med-VQA). However, most existing models focus on global image features rather than localizing disease-specific regions crucial for diagnosis. Additionally, current research tends to emphasize answer accuracy at the expense of the reasoning pathway, yet both are crucial for clinical decision-making. To address these challenges, we propose From Vision to Text Chain-of-Thought (V2T-CoT), a novel approach that automates the localization of preference areas within biomedical images and incorporates this localization into region-level pixel attention as knowledge for Vision CoT. By fine-tuning the vision language model on constructed R-Med 39K dataset, V2T-CoT provides definitive medical reasoning paths. V2T-CoT integrates visual grounding with textual rationale generation to establish precise and explainable diagnostic results. Experimental results across four Med-VQA benchmarks demonstrate state-of-the-art performance, achieving substantial improvements in both performance and interpretability.

Med-U1: Incentivizing Unified Medical Reasoning in LLMs via Large-scale Reinforcement Learning

Medical Question-Answering (QA) encompasses a broad spectrum of tasks, including multiple choice questions (MCQ), open-ended text generation, and complex computational reasoning. Despite this variety, a unified framework for delivering high-quality medical QA has yet to emerge. Although recent progress in reasoning-augmented large language models (LLMs) has shown promise, their ability to achieve comprehensive medical understanding is still largely unexplored. In this paper, we present Med-U1, a unified framework for robust reasoning across medical QA tasks with diverse output formats, ranging from MCQs to complex generation and computation tasks. Med-U1 employs pure large-scale reinforcement learning with mixed rule-based binary reward functions, incorporating a length penalty to manage output verbosity. With multi-objective reward optimization, Med-U1 directs LLMs to produce concise and verifiable reasoning chains. Empirical results reveal that Med-U1 significantly improves performance across multiple challenging Med-QA benchmarks, surpassing even larger specialized and proprietary models. Furthermore, Med-U1 demonstrates robust generalization to out-of-distribution (OOD) tasks. Extensive analysis presents insights into training strategies, reasoning chain length control, and reward design for medical LLMs. Our code is available here.

CAPO: Reinforcing Consistent Reasoning in Medical Decision-Making

In medical visual question answering (Med-VQA), achieving accurate responses relies on three critical steps: precise perception of medical imaging data, logical reasoning grounded in visual input and textual questions, and coherent answer derivation from the reasoning process. Recent advances in general vision-language models (VLMs) show that large-scale reinforcement learning (RL) could significantly enhance both reasoning capabilities and overall model performance. However, their application in medical domains is hindered by two fundamental challenges: 1) misalignment between perceptual understanding and reasoning stages, and 2) inconsistency between reasoning pathways and answer generation, both compounded by the scarcity of high-quality medical datasets for effective large-scale RL. In this paper, we first introduce Med-Zero-17K, a curated dataset for pure RL-based training, encompassing over 30 medical image modalities and 24 clinical tasks. Moreover, we propose a novel large-scale RL framework for Med-VLMs, Consistency-Aware Preference Optimization (CAPO), which integrates rewards to ensure fidelity between perception and reasoning, consistency in reasoning-to-answer derivation, and rule-based accuracy for final responses. Extensive experiments on both in-domain and out-of-domain scenarios demonstrate the superiority of our method over strong VLM baselines, showcasing strong generalization capability to 3D Med-VQA benchmarks and R1-like training paradigms.
