MeddeX: A Retrieval-Augmented LLaMA-3 System with Gemini Vision for Evidence-Backed Medical Report Analysis

Abstract

Modern healthcare generates increasingly complex medical reports that challenge clinical professionals in terms of time and interpretive burden. Manual review processes are slow, error-prone, and difficult to scale. This paper presents MeddeX, a Medical Report Analyzer that combines a QLoRA fine-tuned LLaMA-3 (8B) model with Retrieval-Augmented Gen eration (RAG) and a multimodal Gemini Vision layer to deliver accurate, concise, and evidence-backed summaries of medical documents. The system handles text-native PDFs, scanned image based PDFs, and direct radiological images through an intelligent hybrid extraction pipeline that includes a novel garbage-text detection mechanism for corrupted and UTF-16-encoded doc uments. Beyond single-report Q&A, MeddeX introduces report scoped retrieval for precise contextual grounding, an automated longitudinal report comparison engine, and a self-healing backfill pipeline that lazily processes legacy documents on demand. A FastAPI backend, Supabase vector store, and Next.js fron tend complete the end-to-end architecture. Evaluation against the base LLaMA3-Med42-8B model demonstrates consistent improvements across ROUGE-1 (+9.2%), ROUGE-2 (+10.0%), ROUGE-L (+11.8%), BLEU (+71.4%), and BERTScore (+0.8%), validating the benefit of domain-specific fine-tuning. MeddeX represents a practical and robust step toward AI-assisted clinical decision support.

VietMed: A Dataset and Benchmark for Automatic Speech Recognition of Vietnamese in the Medical Domain

Due to privacy restrictions, there's a shortage of publicly available speech recognition datasets in the medical domain. In this work, we present VietMed - a Vietnamese speech recognition dataset in the medical domain comprising 16h of labeled medical speech, 1000h of unlabeled medical speech and 1200h of unlabeled general-domain speech. To our best knowledge, VietMed is by far the world's largest public medical speech recognition dataset in 7 aspects: total duration, number of speakers, diseases, recording conditions, speaker roles, unique medical terms and accents. VietMed is also by far the largest public Vietnamese speech dataset in terms of total duration. Additionally, we are the first to present a medical ASR dataset covering all ICD-10 disease groups and all accents within a country. Moreover, we release the first public large-scale pre-trained models for Vietnamese ASR, w2v2-Viet and XLSR-53-Viet, along with the first public large-scale fine-tuned models for medical ASR. Even without any medical data in unsupervised pre-training, our best pre-trained model XLSR-53-Viet generalizes very well to the medical domain by outperforming state-of-the-art XLSR-53, from 51.8% to 29.6% WER on test set (a relative reduction of more than 40%). Token-Budget-Aware LLM Reasoning

Reasoning is critical for large language models (LLMs) to excel in a wide range of tasks. While methods like Chain-of-Thought (CoT) reasoning and enhance LLM performance by decomposing problems into intermediate steps, they also incur significant overhead in token usage, leading to increased costs. We find that the reasoning process of current LLMs is unnecessarily lengthy and it can be compressed by including a reasonable token budget in the prompt, but the choice of token budget plays a crucial role in the actual compression effectiveness. We then propose a token-budget-aware LLM reasoning framework that dynamically adjusts the number of reasoning tokens based on the reasoning complexity of each problem. Experiments show that our method effectively reduces token costs in CoT reasoning with only a slight performance reduction, offering a practical solution to balance efficiency and accuracy in LLM reasoning. Learning to Retrieve and Reason on Knowledge Graph through Active Self-Reflection

Extensive research has investigated the integration of large language models (LLMs) with knowledge graphs to enhance the reasoning process. However, understanding how models perform reasoning utilizing structured graph knowledge remains underexplored. Most existing approaches rely on LLMs or retrievers to make binary judgments regarding the utilization of knowledge, which is too coarse. Meanwhile, there is still a lack of feedback mechanisms for reflection and control throughout the entire reasoning path. This paper proposes an Active self-Reflection framework for knowledge Graph reasoning (ArG), introducing for the first time an end-to-end training approach to achieve iterative reasoning grounded on structured graphs. Within the framework, the model leverages special tokens to actively control the reasoning process, achieving on-demand retrieval and multi-dimensional, finer-grained knowledge evaluation. The reasoning paths generated by the model exhibit high interpretability, enabling deeper exploration of the model's understanding of structured knowledge. Ultimately, the proposed model achieves outstanding results compared to existing baselines in knowledge graph reasoning tasks.
