ClinFusion: A Vision-Centric Multimodal LLM System for Holistic Medical Understanding

Multimodal large language models (MLLMs) hold immense potential to revolutionize clinical practice, yet deploying them in the medical domain is fundamentally a vision-centric challenge: models must absorb knowledge from heterogeneous 2D and 3D medical images, and evaluation protocols must align with radiologists' clinical practice and provide an accurate, fine-grained and factualness-driven assessment. In this paper, we introduce ClinFusion, a vision-centric MLLM designed for holistic medical understanding that systematically addresses these limitations. We propose a compositional and cascaded vision encoder architecture featuring a Cascade Spatial-Aware Locality Fusion operator that unifies diverse 2D and native 3D medical image understanding within a fused encoder. We further introduce a vision-grounded evaluation framework, including MedIF-Bench for instruction-following assessment and a region-of-interest-grounded method for clinically aligned and factualness-driven report generation evaluation. We show that ClinFusion sets a new state-of-the-art across a comprehensive suite of 2D and 3D multimodal medical benchmarks---spanning visual question answering, report generation, and instruction following---as well as textual medical tasks, outperforming leading open-source medical MLLMs (\textit{e.g.}, Hulu-Med, Lingshu) on 20 out of 24 benchmarks and demonstrating multimodal capabilities better than powerful proprietary models such as GPT-5.2 and Gemini-3-Flash on 13 out of 16 benchmarks, and can be further augmented with agentic tool use for retrieval-augmented and tool-assisted clinical workflows. A blinded evaluation by board-certified radiologists confirms that ClinFusion produces the highest-ranked reports, and validates our RoI-grounded metric as achieving the strongest correlation with expert judgment among all automatic evaluation metrics examined.

Unified Hallucination Fuzzing for Multimodal Large Language Models

Hallucination remains a persistent challenge for Multimodal Large Language Models (MLLMs), severely limiting their reliability in high-stakes applications. Existing evaluations, predominantly based on static benchmarks, suffer from narrow taxonomical coverage and rapid performance saturation, failing to reflect model robustness in evolving real-world scenarios. To bridge this gap, we present a systematic evaluation framework integrating a comprehensive benchmark with self-evolving stress testing. First, we introduce UniHall, a fine-grained dataset grounded in a unified taxonomy spanning Object, Instruction, and Knowledge dimensions. Second, to address benchmark saturation, we propose Self-Adaptive Multimodal Fuzzing (SAMF), a self-adaptive framework that employs evolutionary mutation strategies to explore the boundaries of model hallucinations. Crucially, to ensure reliable assessment of dynamic inputs, SAMF incorporates a structured metric suite driven by an ensemble of multi-modal oracles. Our extensive experiments reveal that state-of-the-art MLLMs exhibit significant performance degradation under fuzzing compared to conventional settings, exposing a dissociation between reasoning capabilities and factual grounding. Furthermore, we identify a helpfulness-hallucination trade-off, where reinforcement learning alignment inadvertently exacerbates sycophancy in instruction-following tasks. The framework, code and benchmark are available at https://github.com/LanceZPF/EvalHall.

Towards Error-Free Long Video Generation

Recent advances in video generation have made minute-level synthesis possible; however, generating long videos remains challenging due to error accumulation, attribute drift, and the limited availability of long video data. In this paper, we introduce an infinite-length video generation framework that focusing on addressing these issues and produces high-quality, dynamic, and identity-consistent single-shot long videos. We first finetune a diffusion model as a video extension model on large-scale short video data to autoregressively generate temporally coherent clips. Inspired by the success of large language models (LLMs), we adopt causal attention computation between clips to further finetune this model on long video data. In this way, the tokens in one clip (short video) are computed by bidirectional attention while tokens among clips are computed by unidirectional attention. This design leverages the strengths of modern diffusion models while preserving long-term context information, effectively mitigating error accumulation and attribute drift. To achieve memory efficiency during inference, we adopt a key-value (KV) caching mechanism to maintain a constant KV memory. Furthermore, we introduce truncation-rectified flow (T-RFlow) technique to further suppress error accumulation. Experimental results demonstrate the effectiveness of our method. Our framework establishes a new benchmark for realistic and coherent minute-level video synthesis.

ClinHallu: A Benchmark for Diagnosing Stage-Wise Hallucinations in Medical MLLM Reasoning

Building trustworthy medical multimodal large language models (MLLMs) is critical for reliable clinical decision support. Existing medical hallucination benchmarks mainly focus on data collection, but often ignore where hallucinations originate within the reasoning process. We find that hallucination sources vary across samples: errors may arise from visual misrecognition, incorrect medical knowledge recall, or flawed reasoning integration. To enable source-level hallucination diagnosis, we introduce ClinHallu, a benchmark for stage-wise hallucination diagnosis in medical MLLM reasoning. ClinHallu contains 7,031 validated instances, where each instance is augmented with a structured reasoning trace decomposed into Visual Recognition, Knowledge Recall, and Reasoning Integration. We also use stage-replacement interventions to measure how correcting specific stages affects the final answer. Beyond evaluation, we show that trace-supervised fine-tuning reduces stage-wise hallucinations. ClinHallu provides a fine-grained hallucination testbed for diagnosing and mitigating reasoning failures in medical MLLMs. The benchmark is publicly available at https://github.com/alibaba-damo-academy/ClinHallu.

Towards 3D-Aware Video Diffusion Models: Render-Free Human Motion Control with Mesh Tokenization

Diffusion models have shown remarkable success in video generation. However, whether such models are truly aware of the 3D structure underlying visual observations, rather than simply reproducing plausible 2D projections, remains an open question. In this work, we investigate this question through human motion control, a task that requires precise modelling of 3D human geometry, motion, camera viewpoint, and scene context. Unlike prior methods that rely on rendered 2D motion guidance videos, we propose a render-free framework that conditions video generation directly on compressed 3D human mesh tokens. This representation preserves full 3D geometric information while enabling a unified token-based generation pipeline that processes video tokens jointly with motion tokens in a DiT-based architecture. This design requires the model to reason jointly about appearance, 3D structure, and camera viewpoint during video generation. Experimental results demonstrate strong performance on human motion control benchmarks, while reducing artifacts induced by view-dependent 2D guidance and trajectory-pose mismatches during editing. These findings suggest that video diffusion models, when equipped with mesh tokenization, can better capture complex 3D human structures and their interactions with the surrounding environment.

Lumos-Nexus: Efficient Frequency Bridging with Homogeneous Latent Space for Video Unified Models

Connector-based video unified models have demonstrated strong capability in instruction-grounded video synthesis, but integrating a large high-fidelity generator into the unified training loop is computationally prohibitive, limiting achievable visual quality. We therefore propose Lumos-Nexus, a training-efficient unified video generation framework that facilitates the development of strong reasoning-driven generation capabilities while significantly enhancing visual fidelity. Lumos-Nexus adopts a two-stage design: 1) During training, only a lightweight generator is aligned with the understanding block to learn to take in reasoning-driven semantic control. 2) During inference, we introduce Unified Progressive Frequency Bridging (UPFB) to progressively hand off generation to a high-capacity pretrained generator in the shared latent space, enabling coarse-to-fine refinement and producing high-fidelity videos without compromising reasoning quality. To fill the gap in reasoning-driven video generation benchmarks, we introduce VR-Bench, which assesses a model's capability to translate inferred intent into coherent and semantically aligned video content. Extensive experiments demonstrate that Lumos-Nexus achieves substantial gains in visual realism and temporal coherence on VBench, while exhibiting strong reasoning-based generative performance on VR-Bench. LumosX: Relate Any Identities with Their Attributes for Personalized Video Generation

Recent advances in diffusion models have significantly improved text-to-video generation, enabling personalized content creation with fine-grained control over both foreground and background elements. However, precise face-attribute alignment across subjects remains challenging, as existing methods lack explicit mechanisms to ensure intra-group consistency. Addressing this gap requires both explicit modeling strategies and face-attribute-aware data resources. We therefore propose LumosX, a framework that advances both data and model design. On the data side, a tailored collection pipeline orchestrates captions and visual cues from independent videos, while multimodal large language models (MLLMs) infer and assign subject-specific dependencies. These extracted relational priors impose a finer-grained structure that amplifies the expressive control of personalized video generation and enables the construction of a comprehensive benchmark. On the modeling side, Relational Self-Attention and Relational Cross-Attention intertwine position-aware embeddings with refined attention dynamics to inscribe explicit subject-attribute dependencies, enforcing disciplined intra-group cohesion and amplifying the separation between distinct subject clusters. Comprehensive evaluations on our benchmark demonstrate that LumosX achieves state-of-the-art performance in fine-grained, identity-consistent, and semantically aligned personalized multi-subject video generation. Lumos-1: On Autoregressive Video Generation with Discrete Diffusion from a Unified Model Perspective

Autoregressive large language models (LLMs) have unified a vast range of language tasks, inspiring preliminary efforts in autoregressive (AR) video generation. Existing AR video generators either diverge from standard LLM architectures, depend on bulky external text encoders, or incur prohibitive latency due to next-token decoding. In this paper, we introduce Lumos-1, an LLM-based unified model for AR video generation with efficient discrete diffusion. Firstly, to fit videos with LLMs, we identify that 1D RoPE is ill-suited for visual spatiotemporal correlation modeling, and while demonstrated to be useful, naive 3D RoPE exhibits imbalanced frequency spectra. Therefore, we propose MM-RoPE, which preserves the original textual RoPE while seamlessly accommodating video data with comprehensive frequency spectra and scaled 3D positions. Secondly, to fit the video data's nature and overcome the inefficiency of next-token decoding, we adopt a parallel and mask-based discrete diffusion with the intra-frame bidirectional and inter-frame causal attention masks. Based on this attention mask, we uncover the frame-wise loss imbalance issue caused by spatial information redundancy and propose Autoregressive Discrete Diffusion Forcing, which introduces temporal tube masking during training with a compatible inference-time masking policy to avoid quality degradation. Despite using only 48 GPUs for pre-training and fine-tuning, limited data and a discrete tokenizer, Lumos-1 achieves results surpassing those of Show-o2 on GenEval, COSMOS-Video2World on VBench-I2V, and OpenSoraPlan on VBench-T2V. MovieDreamer: Hierarchical Generation for Coherent Long Visual Sequence

Recent advancements in video generation have primarily leveraged diffusion models for short-duration content. However, these approaches often fall short in modeling complex narratives and maintaining character consistency over extended periods, which is essential for long-form video production like movies. We propose MovieDreamer, a novel hierarchical framework that integrates the strengths of autoregressive models with diffusion-based rendering to pioneer long-duration video generation with intricate plot progressions and high visual fidelity. Our approach utilizes autoregressive models for global narrative coherence, predicting sequences of visual tokens that are subsequently transformed into high-quality video frames through diffusion rendering. This method is akin to traditional movie production processes, where complex stories are factorized down into manageable scene capturing. Further, we employ a multimodal script that enriches scene descriptions with detailed character information and visual style, enhancing continuity and character identity across scenes. We present extensive experiments across various movie genres, demonstrating that our approach not only achieves superior visual and narrative quality but also effectively extends the duration of generated content significantly beyond current capabilities. UniLumos: Fast and Unified Image and Video Relighting with Physics-Plausible Feedback

Relighting is a crucial task with both practical demand and artistic value, and recent diffusion models have shown strong potential by enabling rich and controllable lighting effects. However, as they are typically optimized in semantic latent space, where proximity does not guarantee physical correctness in visual space, they often produce unrealistic results, such as overexposed highlights, misaligned shadows, and incorrect occlusions. We address this with UniLumos, a unified relighting framework for both images and videos that brings RGB-space geometry feedback into a flow matching backbone. By supervising the model with depth and normal maps extracted from its outputs, we explicitly align lighting effects with the scene structure, enhancing physical plausibility. Nevertheless, this feedback requires high-quality outputs for supervision in visual space, making standard multi-step denoising computationally expensive. To mitigate this, we employ path consistency learning, allowing supervision to remain effective even under few-step training regimes. To enable fine-grained relighting control and supervision, we design a structured six-dimensional annotation protocol capturing core illumination attributes. Building upon this, we propose LumosBench, a disentangled attribute-level benchmark that evaluates lighting controllability via large vision-language models, enabling automatic and interpretable assessment of relighting precision across individual dimensions. Extensive experiments demonstrate that UniLumos achieves state-of-the-art relighting quality with significantly improved physical consistency, while delivering a 20x speedup for both image and video relighting. Every Step Evolves: Scaling Reinforcement Learning for Trillion-Scale Thinking Model

We present Ring-1T, the first open-source, state-of-the-art thinking model with a trillion-scale parameter. It features 1 trillion total parameters and activates approximately 50 billion per token. Training such models at a trillion-parameter scale introduces unprecedented challenges, including train-inference misalignment, inefficiencies in rollout processing, and bottlenecks in the RL system. To address these, we pioneer three interconnected innovations: (1) IcePop stabilizes RL training via token-level discrepancy masking and clipping, resolving instability from training-inference mismatches; (2) C3PO++ improves resource utilization for long rollouts under a token budget by dynamically partitioning them, thereby obtaining high time efficiency; and (3) ASystem, a high-performance RL framework designed to overcome the systemic bottlenecks that impede trillion-parameter model training. Ring-1T delivers breakthrough results across critical benchmarks: 93.4 on AIME-2025, 86.72 on HMMT-2025, 2088 on CodeForces, and 55.94 on ARC-AGI-1. Notably, it attains a silver medal-level result on the IMO-2025, underscoring its exceptional reasoning capabilities. By releasing the complete 1T parameter MoE model to the community, we provide the research community with direct access to cutting-edge reasoning capabilities. This contribution marks a significant milestone in democratizing large-scale reasoning intelligence and establishes a new baseline for open-source model performance.

The 1st Solution for CARE Liver Task Challenge 2025: Contrast-Aware Semi-Supervised Segmentation with Domain Generalization and Test-Time Adaptation

Accurate liver segmentation from contrast-enhanced MRI is essential for diagnosis, treatment planning, and disease monitoring. However, it remains challenging due to limited annotated data, heterogeneous enhancement protocols, and significant domain shifts across scanners and institutions. Traditional image-to-image translation frameworks have made great progress in domain generalization, but their application is not straightforward. For example, Pix2Pix requires image registration, and cycle-GAN cannot be integrated seamlessly into segmentation pipelines. Meanwhile, these methods are originally used to deal with cross-modality scenarios, and often introduce structural distortions and suffer from unstable training, which may pose drawbacks in our single-modality scenario. To address these challenges, we propose CoSSeg-TTA, a compact segmentation framework for the GED4 (Gd-EOB-DTPA enhanced hepatobiliary phase MRI) modality built upon nnU-Netv2 and enhanced with a semi-supervised mean teacher scheme to exploit large amounts of unlabeled volumes. A domain adaptation module, incorporating a randomized histogram-based style appearance transfer function and a trainable contrast-aware network, enriches domain diversity and mitigates cross-center variability. Furthermore, a continual test-time adaptation strategy is employed to improve robustness during inference. Extensive experiments demonstrate that our framework consistently outperforms the nnU-Netv2 baseline, achieving superior Dice score and Hausdorff Distance while exhibiting strong generalization to unseen domains under low-annotation conditions.

RealisMotion: Decomposed Human Motion Control and Video Generation in the World Space

Generating human videos with realistic and controllable motions is a challenging task. While existing methods can generate visually compelling videos, they lack separate control over four key video elements: foreground subject, background video, human trajectory and action patterns. In this paper, we propose a decomposed human motion control and video generation framework that explicitly decouples motion from appearance, subject from background, and action from trajectory, enabling flexible mix-and-match composition of these elements. Concretely, we first build a ground-aware 3D world coordinate system and perform motion editing directly in the 3D space. Trajectory control is implemented by unprojecting edited 2D trajectories into 3D with focal-length calibration and coordinate transformation, followed by speed alignment and orientation adjustment; actions are supplied by a motion bank or generated via text-to-motion methods. Then, based on modern text-to-video diffusion transformer models, we inject the subject as tokens for full attention, concatenate the background along the channel dimension, and add motion (trajectory and action) control signals by addition. Such a design opens up the possibility for us to generate realistic videos of anyone doing anything anywhere. Extensive experiments on benchmark datasets and real-world cases demonstrate that our method achieves state-of-the-art performance on both element-wise controllability and overall video quality.

MoDA: Multi-modal Diffusion Architecture for Talking Head Generation

Talking head generation with arbitrary identities and speech audio remains a crucial problem in the realm of the virtual metaverse. Recently, diffusion models have become a popular generative technique in this field with their strong generation capabilities. However, several challenges remain for diffusion-based methods: 1) inefficient inference and visual artifacts caused by the implicit latent space of Variational Auto-Encoders (VAE), which complicates the diffusion process; 2) a lack of authentic facial expressions and head movements due to inadequate multi-modal information fusion. In this paper, MoDA handles these challenges by: 1) defining a joint parameter space that bridges motion generation and neural rendering, and leveraging flow matching to simplify diffusion learning; 2) introducing a multi-modal diffusion architecture to model the interaction among noisy motion, audio, and auxiliary conditions, enhancing overall facial expressiveness. In addition, a coarse-to-fine fusion strategy is employed to progressively integrate different modalities, ensuring effective feature fusion. Experimental results demonstrate that MoDA improves video diversity, realism, and efficiency, making it suitable for real-world applications. RealisVSR: Detail-enhanced Diffusion for Real-World 4K Video Super-Resolution

Video Super-Resolution (VSR) has achieved significant progress through diffusion models, effectively addressing the over-smoothing issues inherent in GAN-based methods. Despite recent advances, three critical challenges persist in VSR community: 1) Inconsistent modeling of temporal dynamics in foundational models; 2) limited high-frequency detail recovery under complex real-world degradations; and 3) insufficient evaluation of detail enhancement and 4K super-resolution, as current methods primarily rely on 720P datasets with inadequate details. To address these challenges, we propose RealisVSR, a high-frequency detail-enhanced video diffusion model with three core innovations: 1) Consistency Preserved ControlNet (CPC) architecture integrated with the Wan2.1 video diffusion to model the smooth and complex motions and suppress artifacts; 2) High-Frequency Rectified Diffusion Loss (HR-Loss) combining wavelet decomposition and HOG feature constraints for texture restoration; 3) RealisVideo-4K, the first public 4K VSR benchmark containing 1,000 high-definition video-text pairs. Leveraging the advanced spatio-temporal guidance of Wan2.1, our method requires only 5-25% of the training data volume compared to existing approaches. Extensive experiments on VSR benchmarks (REDS, SPMCS, UDM10, YouTube-HQ, VideoLQ, RealisVideo-720P) demonstrate our superiority, particularly in ultra-high-resolution scenarios.

CoT-lized Diffusion: Let's Reinforce T2I Generation Step-by-step

Current text-to-image (T2I) generation models struggle to align spatial composition with the input text, especially in complex scenes. Even layout-based approaches yield suboptimal spatial control, as their generation process is decoupled from layout planning, making it difficult to refine the layout during synthesis. We present CoT-Diff, a framework that brings step-by-step CoT-style reasoning into T2I generation by tightly integrating Multimodal Large Language Model (MLLM)-driven 3D layout planning with the diffusion process. CoT-Diff enables layout-aware reasoning inline within a single diffusion round: at each denoising step, the MLLM evaluates intermediate predictions, dynamically updates the 3D scene layout, and continuously guides the generation process. The updated layout is converted into semantic conditions and depth maps, which are fused into the diffusion model via a condition-aware attention mechanism, enabling precise spatial control and semantic injection. Experiments on 3D Scene benchmarks show that CoT-Diff significantly improves spatial alignment and compositional fidelity, and outperforms the state-of-the-art method by 34.7% in complex scene spatial accuracy, thereby validating the effectiveness of this entangled generation paradigm.

Frequency-Resolved Forward Capacitance in GaN-based LEDs

This study establishes a unified framework for interpreting dynamic capacitive responses in InGaN-based light-emitting diodes (LEDs) through forward-bias capacitance-voltage-frequency spectroscopy. A hybrid impedance model integrating series RL components and parallel C-G networks was developed to resolve distinct frequency-dependent capacitive regimes. The low-frequency regime (<1 kHz) is governed by interfacial capacitance with characteristic reciprocal frequency dependence, while the mid-frequency range(10 kHz-6.4 MHz) demonstrates carrier diffusion and recombination dynamics. At MHz frequencies, negative capacitance manifests due to delayed carrier emission mediated by deep-level traps. The model achieved sub-1% fitting errors (R^2 > 0.99)across a broad bandwidth(10 kHz-6.4 MHz) , conclusively attributing negative capacitance to intrinsic trap processes rather than extrinsic artifacts. Critical advances include quantum well cap thickness modulation reducing mid-frequency capacitance by 30% and the dominance of trap-mediated inductance over parasitic contributions by three orders of magnitude. This framework resolves persistent controversies in LED impedance interpretation. By bridging semiconductor physics with device engineering, this methodology provides essential tools for designing next-generation optoelectronic systems requiring ultralow-latency operation and precise charge-state control.

FPSAttention: Training-Aware FP8 and Sparsity Co-Design for Fast Video Diffusion

Diffusion generative models have become the standard for producing high-quality, coherent video content, yet their slow inference speeds and high computational demands hinder practical deployment. Although both quantization and sparsity can independently accelerate inference while maintaining generation quality, naively combining these techniques in existing training-free approaches leads to significant performance degradation due to the lack of joint optimization. We introduce FPSAttention, a novel training-aware co-design of FP8 quantization and sparsity for video generation, with a focus on the 3D bi-directional attention mechanism. Our approach features three key innovations: 1) A unified 3D tile-wise granularity that simultaneously supports both quantization and sparsity; 2) A denoising step-aware strategy that adapts to the noise schedule, addressing the strong correlation between quantization/sparsity errors and denoising steps; 3) A native, hardware-friendly kernel that leverages FlashAttention and is implemented with optimized Hopper architecture features for highly efficient execution. Trained on Wan2.1's 1.3B and 14B models and evaluated on the VBench benchmark, FPSAttention achieves a 7.09x kernel speedup for attention operations and a 4.96x end-to-end speedup for video generation compared to the BF16 baseline at 720p resolution-without sacrificing generation quality.
