Context Tokens are Anchors: Understanding the Repetition Curse in dMLLMs from an Information Flow Perspective

Recent diffusion-based Multimodal Large Language Models (dMLLMs) suffer from high inference latency and therefore rely on caching techniques to accelerate decoding. However, the application of cache mechanisms often introduces undesirable repetitive text generation, a phenomenon we term the \textbf{Repeat Curse}. To better investigate underlying mechanism behind this issue, we analyze repetition generation through the lens of information flow. Our work reveals three key findings: (1) context tokens aggregate semantic information as anchors and guide the final predictions; (2) as information propagates across layers, the entropy of context tokens converges in deeper layers, reflecting the model's growing prediction certainty; (3) Repetition is typically linked to disruptions in the information flow of context tokens and to the inability of their entropy to converge in deeper layers. Based on these insights, we present \textbf{CoTA}, a plug-and-play method for mitigating repetition. CoTA enhances the attention of context tokens to preserve intrinsic information flow patterns, while introducing a penalty term to the confidence score during decoding to avoid outputs driven by uncertain context tokens. With extensive experiments, CoTA demonstrates significant effectiveness in alleviating repetition and achieves consistent performance improvements on general tasks. ProMist-5K: A Comprehensive Dataset for Digital Emulation of Cinematic Pro-Mist Filter Effects

Pro-Mist filters are widely used in cinematography for their ability to create soft halation, lower contrast, and produce a distinctive, atmospheric style. These effects are difficult to reproduce digitally due to the complex behavior of light diffusion. We present ProMist-5K, a dataset designed to support cinematic style emulation. It is built using a physically inspired pipeline in a scene-referred linear space and includes 20,000 high-resolution image pairs across four configurations, covering two filter densities (1/2 and 1/8) and two focal lengths (20mm and 50mm). Unlike general style datasets, ProMist-5K focuses on realistic glow and highlight diffusion effects. Multiple blur layers and carefully tuned weighting are used to model the varying intensity and spread of optical diffusion. The dataset provides a consistent and controllable target domain that supports various image translation models and learning paradigms. Experiments show that the dataset works well across different training settings and helps capture both subtle and strong cinematic appearances. ProMist-5K offers a practical and physically grounded resource for film-inspired image transformation, bridging the gap between digital flexibility and traditional lens aesthetics. Beyond Shadows: A Large-Scale Benchmark and Multi-Stage Framework for High-Fidelity Facial Shadow Removal

Facial shadows often degrade image quality and the performance of vision algorithms. Existing methods struggle to remove shadows while preserving texture, especially under complex lighting conditions, and they lack real-world paired datasets for training. We present the Augmented Shadow Face in the Wild (ASFW) dataset, the first large-scale real-world dataset for facial shadow removal, containing 1,081 paired shadow and shadow-free images created via a professional Photoshop workflow. ASFW offers photorealistic shadow variations and accurate ground truths, bridging the gap between synthetic and real domains. Deep models trained on ASFW demonstrate improved shadow removal in real-world conditions. We also introduce the Face Shadow Eraser (FSE) method to showcase the effectiveness of the dataset. Experiments demonstrate that ASFW enhances the performance of facial shadow removal models, setting new standards for this task.

Wearable-informed generative digital avatars predict task-conditioned post-stroke locomotion

Dynamic prediction of locomotor capacity after stroke could enable more individualized rehabilitation, yet current assessments largely provide static impairment scores and do not indicate whether patients can perform specific tasks such as slope walking or stair climbing. Here, we present a wearable-informed data-physics hybrid generative framework that reconstructs a stroke survivor's locomotor control from wearable inertial sensing and predicts task-conditioned post-stroke locomotion in new environments. From a single 20 m level-ground walking trial recorded by five IMUs, the framework personalizes a physics-based digital avatar using a healthy-motion prior and hybrid imitation learning, generating dynamically feasible, patient-specific movements for inclined walking and stair negotiation. Across 11 stroke inpatients, predicted postures reached 82.2% similarity for slopes and 69.9% for stairs, substantially exceeding a physics-only baseline. In a multicentre pilot randomized study (n = 21; 28 days), access to scenario-specific locomotion predictions to support task selection and difficulty titration was associated with larger gains in Fugl-Meyer lower-extremity scores than standard care (mean change 6.0 vs 3.7 points; $p < 0.05$). These results suggest that wearable-informed generative digital avatars may augment individualized gait rehabilitation planning and provide a pathway toward dynamically personalized post-stroke motor recovery strategies.

HBFormer: A Hybrid-Bridge Transformer for Microtumor and Miniature Organ Segmentation

Medical image segmentation is a cornerstone of modern clinical diagnostics. While Vision Transformers that leverage shifted window-based self-attention have established new benchmarks in this field, they are often hampered by a critical limitation: their localized attention mechanism struggles to effectively fuse local details with global context. This deficiency is particularly detrimental to challenging tasks such as the segmentation of microtumors and miniature organs, where both fine-grained boundary definition and broad contextual understanding are paramount. To address this gap, we propose HBFormer, a novel Hybrid-Bridge Transformer architecture. The 'Hybrid' design of HBFormer synergizes a classic U-shaped encoder-decoder framework with a powerful Swin Transformer backbone for robust hierarchical feature extraction. The core innovation lies in its 'Bridge' mechanism, a sophisticated nexus for multi-scale feature integration. This bridge is architecturally embodied by our novel Multi-Scale Feature Fusion (MFF) decoder. Departing from conventional symmetric designs, the MFF decoder is engineered to fuse multi-scale features from the encoder with global contextual information. It achieves this through a synergistic combination of channel and spatial attention modules, which are constructed from a series of dilated and depth-wise convolutions. These components work in concert to create a powerful feature bridge that explicitly captures long-range dependencies and refines object boundaries with exceptional precision. Comprehensive experiments on challenging medical image segmentation datasets, including multi-organ, liver tumor, and bladder tumor benchmarks, demonstrate that HBFormer achieves state-of-the-art results, showcasing its outstanding capabilities in microtumor and miniature organ segmentation. Adaptive Query Prompting for Multi-Domain Landmark Detection

Medical landmark detection is crucial in various medical imaging modalities and procedures. Although deep learning-based methods have achieve promising performance, they are mostly designed for specific anatomical regions or tasks. In this work, we propose a universal model for multi-domain landmark detection by leveraging transformer architecture and developing a prompting component, named as Adaptive Query Prompting (AQP). Instead of embedding additional modules in the backbone network, we design a separate module to generate prompts that can be effectively extended to any other transformer network. In our proposed AQP, prompts are learnable parameters maintained in a memory space called prompt pool. The central idea is to keep the backbone frozen and then optimize prompts to instruct the model inference process. Furthermore, we employ a lightweight decoder to decode landmarks from the extracted features, namely Light-MLD. Thanks to the lightweight nature of the decoder and AQP, we can handle multiple datasets by sharing the backbone encoder and then only perform partial parameter tuning without incurring much additional cost. It has the potential to be extended to more landmark detection tasks. We conduct experiments on three widely used X-ray datasets for different medical landmark detection tasks. Our proposed Light-MLD coupled with AQP achieves SOTA performance on many metrics even without the use of elaborate structural designs or complex frameworks.

Tag-Enriched Multi-Attention with Large Language Models for Cross-Domain Sequential Recommendation

Cross-Domain Sequential Recommendation (CDSR) plays a crucial role in modern consumer electronics and e-commerce platforms, where users interact with diverse services such as books, movies, and online retail products. These systems must accurately capture both domain-specific and cross-domain behavioral patterns to provide personalized and seamless consumer experiences. To address this challenge, we propose \textbf{TEMA-LLM} (\textit{Tag-Enriched Multi-Attention with Large Language Models}), a practical and effective framework that integrates \textit{Large Language Models (LLMs)} for semantic tag generation and enrichment. Specifically, TEMA-LLM employs LLMs to assign domain-aware prompts and generate descriptive tags from item titles and descriptions. The resulting tag embeddings are fused with item identifiers as well as textual and visual features to construct enhanced item representations. A \textit{Tag-Enriched Multi-Attention} mechanism is then introduced to jointly model user preferences within and across domains, enabling the system to capture complex and evolving consumer interests. Extensive experiments on four large-scale e-commerce datasets demonstrate that TEMA-LLM consistently outperforms state-of-the-art baselines, underscoring the benefits of LLM-based semantic tagging and multi-attention integration for consumer-facing recommendation systems. The proposed approach highlights the potential of LLMs to advance intelligent, user-centric services in the field of consumer electronics.

An AI-Driven Multimodal Smart Home Platform for Continuous Monitoring and Assistance in Post-Stroke Motor Impairment

At-home rehabilitation for post-stroke patients presents significant challenges, as continuous, personalized care is often limited outside clinical settings. Moreover, the lack of integrated solutions capable of simultaneously monitoring motor recovery and providing intelligent assistance in home environments hampers rehabilitation outcomes. Here, we present a multimodal smart home platform designed for continuous, at-home rehabilitation of post-stroke patients, integrating wearable sensing, ambient monitoring, and adaptive automation. A plantar pressure insole equipped with a machine learning pipeline classifies users into motor recovery stages with up to 94\% accuracy, enabling quantitative tracking of walking patterns during daily activities. An optional head-mounted eye-tracking module, together with ambient sensors such as cameras and microphones, supports seamless hands-free control of household devices with a 100\% success rate and sub-second response time. These data streams are fused locally via a hierarchical Internet of Things (IoT) architecture, ensuring low latency and data privacy. An embedded large language model (LLM) agent, Auto-Care, continuously interprets multimodal data to provide real-time interventions -- issuing personalized reminders, adjusting environmental conditions, and notifying caregivers. Implemented in a post-stroke context, this integrated smart home platform increased mean user satisfaction from 3.9 $\pm$ 0.8 in conventional home environments to 8.4 $\pm$ 0.6 with the full system ($n=20$). Beyond stroke, the system offers a scalable, patient-centered framework with potential for long-term use in broader neurorehabilitation and aging-in-place applications.

Elevating Medical Image Security: A Cryptographic Framework Integrating Hyperchaotic Map and GRU

Chaotic systems play a key role in modern image encryption due to their sensitivity to initial conditions, ergodicity, and complex dynamics. However, many existing chaos-based encryption methods suffer from vulnerabilities, such as inadequate permutation and diffusion, and suboptimal pseudorandom properties. This paper presents Kun-IE, a novel encryption framework designed to address these issues. The framework features two key contributions: the development of the 2D Sin-Cos Pi Hyperchaotic Map (2D-SCPHM), which offers a broader chaotic range and superior pseudorandom sequence generation, and the introduction of Kun-SCAN, a novel permutation strategy that significantly reduces pixel correlations, enhancing resistance to statistical attacks. Kun-IE is flexible and supports encryption for images of any size. Experimental results and security analyses demonstrate its robustness against various cryptanalytic attacks, making it a strong solution for secure image communication. DTEA: Dynamic Topology Weaving and Instability-Driven Entropic Attenuation for Medical Image Segmentation

In medical image segmentation, skip connections are used to merge global context and reduce the semantic gap between encoder and decoder. Current methods often struggle with limited structural representation and insufficient contextual modeling, affecting generalization in complex clinical scenarios. We propose the DTEA model, featuring a new skip connection framework with the Semantic Topology Reconfiguration (STR) and Entropic Perturbation Gating (EPG) modules. STR reorganizes multi-scale semantic features into a dynamic hypergraph to better model cross-resolution anatomical dependencies, enhancing structural and semantic representation. EPG assesses channel stability after perturbation and filters high-entropy channels to emphasize clinically important regions and improve spatial attention. Extensive experiments on three benchmark datasets show our framework achieves superior segmentation accuracy and better generalization across various clinical settings. EEMS: Edge-Prompt Enhanced Medical Image Segmentation Based on Learnable Gating Mechanism

Medical image segmentation is vital for diagnosis, treatment planning, and disease monitoring but is challenged by complex factors like ambiguous edges and background noise. We introduce EEMS, a new model for segmentation, combining an Edge-Aware Enhancement Unit (EAEU) and a Multi-scale Prompt Generation Unit (MSPGU). EAEU enhances edge perception via multi-frequency feature extraction, accurately defining boundaries. MSPGU integrates high-level semantic and low-level spatial features using a prompt-guided approach, ensuring precise target localization. The Dual-Source Adaptive Gated Fusion Unit (DAGFU) merges edge features from EAEU with semantic features from MSPGU, enhancing segmentation accuracy and robustness. Tests on datasets like ISIC2018 confirm EEMS's superior performance and reliability as a clinical tool.

TDADL-IE: A Deep Learning-Driven Cryptographic Architecture for Medical Image Security

The rise of digital medical imaging, like MRI and CT, demands strong encryption to protect patient data in telemedicine and cloud storage. Chaotic systems are popular for image encryption due to their sensitivity and unique characteristics, but existing methods often lack sufficient security. This paper presents the Three-dimensional Diffusion Algorithm and Deep Learning Image Encryption system (TDADL-IE), built on three key elements. First, we propose an enhanced chaotic generator using an LSTM network with a 1D-Sine Quadratic Chaotic Map (1D-SQCM) for better pseudorandom sequence generation. Next, a new three-dimensional diffusion algorithm (TDA) is applied to encrypt permuted images. TDADL-IE is versatile for images of any size. Experiments confirm its effectiveness against various security threats. The code is available at \href{https://github.com/QuincyQAQ/TDADL-IE}{https://github.com/QuincyQAQ/TDADL-IE}.

FS-RWKV: Leveraging Frequency Spatial-Aware RWKV for 3T-to-7T MRI Translation

Ultra-high-field 7T MRI offers enhanced spatial resolution and tissue contrast that enables the detection of subtle pathological changes in neurological disorders. However, the limited availability of 7T scanners restricts widespread clinical adoption due to substantial infrastructure costs and technical demands. Computational approaches for synthesizing 7T-quality images from accessible 3T acquisitions present a viable solution to this accessibility challenge. Existing CNN approaches suffer from limited spatial coverage, while Transformer models demand excessive computational overhead. RWKV architectures offer an efficient alternative for global feature modeling in medical image synthesis, combining linear computational complexity with strong long-range dependency capture. Building on this foundation, we propose Frequency Spatial-RWKV (FS-RWKV), an RWKV-based framework for 3T-to-7T MRI translation. To better address the challenges of anatomical detail preservation and global tissue contrast recovery, FS-RWKV incorporates two key modules: (1) Frequency-Spatial Omnidirectional Shift (FSO-Shift), which performs discrete wavelet decomposition followed by omnidirectional spatial shifting on the low-frequency branch to enhance global contextual representation while preserving high-frequency anatomical details; and (2) Structural Fidelity Enhancement Block (SFEB), a module that adaptively reinforces anatomical structure through frequency-aware feature fusion. Comprehensive experiments on UNC and BNU datasets demonstrate that FS-RWKV consistently outperforms existing CNN-, Transformer-, GAN-, and RWKV-based baselines across both T1w and T2w modalities, achieving superior anatomical fidelity and perceptual quality.

PC-UNet: An Enforcing Poisson Statistics U-Net for Positron Emission Tomography Denoising

Positron Emission Tomography (PET) is crucial in medicine, but its clinical use is limited due to high signal-to-noise ratio doses increasing radiation exposure. Lowering doses increases Poisson noise, which current denoising methods fail to handle, causing distortions and artifacts. We propose a Poisson Consistent U-Net (PC-UNet) model with a new Poisson Variance and Mean Consistency Loss (PVMC-Loss) that incorporates physical data to improve image fidelity. PVMC-Loss is statistically unbiased in variance and gradient adaptation, acting as a Generalized Method of Moments implementation, offering robustness to minor data mismatches. Tests on PET datasets show PC-UNet improves physical consistency and image fidelity, proving its ability to integrate physical information effectively.

SFormer: SNR-guided Transformer for Underwater Image Enhancement from the Frequency Domain

Recent learning-based underwater image enhancement (UIE) methods have advanced by incorporating physical priors into deep neural networks, particularly using the signal-to-noise ratio (SNR) prior to reduce wavelength-dependent attenuation. However, spatial domain SNR priors have two limitations: (i) they cannot effectively separate cross-channel interference, and (ii) they provide limited help in amplifying informative structures while suppressing noise. To overcome these, we propose using the SNR prior in the frequency domain, decomposing features into amplitude and phase spectra for better channel modulation. We introduce the Fourier Attention SNR-prior Transformer (FAST), combining spectral interactions with SNR cues to highlight key spectral components. Additionally, the Frequency Adaptive Transformer (FAT) bottleneck merges low- and high-frequency branches using a gated attention mechanism to enhance perceptual quality. Embedded in a unified U-shaped architecture, these modules integrate a conventional RGB stream with an SNR-guided branch, forming SFormer. Trained on 4,800 paired images from UIEB, EUVP, and LSUI, SFormer surpasses recent methods with a 3.1 dB gain in PSNR and 0.08 in SSIM, successfully restoring colors, textures, and contrast in underwater scenes.

CMAMRNet: A Contextual Mask-Aware Network Enhancing Mural Restoration Through Comprehensive Mask Guidance

Murals, as invaluable cultural artifacts, face continuous deterioration from environmental factors and human activities. Digital restoration of murals faces unique challenges due to their complex degradation patterns and the critical need to preserve artistic authenticity. Existing learning-based methods struggle with maintaining consistent mask guidance throughout their networks, leading to insufficient focus on damaged regions and compromised restoration quality. We propose CMAMRNet, a Contextual Mask-Aware Mural Restoration Network that addresses these limitations through comprehensive mask guidance and multi-scale feature extraction. Our framework introduces two key components: (1) the Mask-Aware Up/Down-Sampler (MAUDS), which ensures consistent mask sensitivity across resolution scales through dedicated channel-wise feature selection and mask-guided feature fusion; and (2) the Co-Feature Aggregator (CFA), operating at both the highest and lowest resolutions to extract complementary features for capturing fine textures and global structures in degraded regions. Experimental results on benchmark datasets demonstrate that CMAMRNet outperforms state-of-the-art methods, effectively preserving both structural integrity and artistic details in restored murals. MCA-LLaVA: Manhattan Causal Attention for Reducing Hallucination in Large Vision-Language Models

Hallucinations pose a significant challenge in Large Vision Language Models (LVLMs), with misalignment between multimodal features identified as a key contributing factor. This paper reveals the negative impact of the long-term decay in Rotary Position Encoding (RoPE), used for positional modeling in LVLMs, on multimodal alignment. Concretely, under long-term decay, instruction tokens exhibit uneven perception of image tokens located at different positions within the two-dimensional space: prioritizing image tokens from the bottom-right region since in the one-dimensional sequence, these tokens are positionally closer to the instruction tokens. This biased perception leads to insufficient image-instruction interaction and suboptimal multimodal alignment. We refer to this phenomenon as image alignment bias. To enhance instruction's perception of image tokens at different spatial locations, we propose MCA-LLaVA, based on Manhattan distance, which extends the long-term decay to a two-dimensional, multi-directional spatial decay. MCA-LLaVA integrates the one-dimensional sequence order and two-dimensional spatial position of image tokens for positional modeling, mitigating hallucinations by alleviating image alignment bias. Experimental results of MCA-LLaVA across various hallucination and general benchmarks demonstrate its effectiveness and generality. The code can be accessed in https://github.com/ErikZ719/MCA-LLaVA.
